Introduction: Pancreatic leak or fistula is the most frequent complication after left pancreatectomy. We performed a single-blinded, parallel-group, randomized controlled trial comparing stapled left pancreatectomy with stapled left pancreatectomy using mesh reinforcement of the staple line with either Seamguard or Peristrips Dry.
Methods: All patients undergoing left pancreatectomy at a large tertiary hospital were eligible for participation. Patients were randomized to either mesh reinforcement or no-mesh reinforcement intraoperatively after being determined a candidate for resection. Patients were blinded to the result of their randomization for 6 weeks. Primary outcome measure was clinically significant leak as defined by the ISGPF (International Study Group on Pancreatic Fistula) pancreatic leak grading system.
Results: One hundred patients were randomized to either mesh (54) or no-mesh (46) reinforcement of their pancreatic transection. There was 1 death in each group. ISGPF grade B and C leaks were seen in 1.9% (1/53) of patients undergoing resection with mesh reinforcement and 20% (11/45) of patients without mesh reinforcement (P = .0007).
Conclusions: Mesh reinforcement of pancreatic transection line significantly reduces the incidence of significant pancreatic fistula in patients undergoing left pancreatectomy.

Aiming at the problems of complex stress and large deformations in the surrounding rocks of the roadway driven under the goafs of small collieries and heading for adjacent advancing coal face, by numerical modeling and field practice, the failure characteristics of the overlying coal and rocks were investigated, and the stopping and resuming times of the roadway excavation were identified. A zoning-based reinforcement technique was put forward and applied in engineering practice. The results showed that (1) The roadway roof was divided into four zones: "Rolid coal zone", "Residual pillar zone", "Roof caved zone", and "Roof un-caved zone".

The research utilized the Coupled Eulerian-Lagrangian (CEL) method for simulation, effectively combining the advantages of both Eulerian and Lagrangian approaches. This research investigates the flexural performance of slabs reinforced with high-strength steel-strand mesh (HSSM) and engineered cementitious composites (ECCs). By employing finite element analysis (FEA) and theoretical modeling, this study aims to deepen the understanding of how these materials behave under bending stresses. A finite element model was developed to simulate the nonlinear behavior of ECCs during bending, considering critical elements such as tensile and compressive damage, as well as bond-slip interactions between the steel strands and the ECCs.
