Given the increases in 5-year cancer survival and recent advances in fertility preserving technologies, an increasing number of women with cancer are presenting for discussion of fertility preserving options. This review will summarize the risk of infertility secondary to cancer treatment, available treatment options for fertility preservation, and techniques to reduce future risks for patients. Concerns that will be addressed include the risk of the medications and procedures, the potential delay in cancer treatment, likelihood of pregnancy complications, as well as the impact of future pregnancy on the recurrence risk of cancer. Recent advances in oocyte cryopreservation and ovarian stimulation protocols will be discussed. Healthcare providers need to be informed of available treatment options including the risks, advantages, and disadvantages of fertility preserving options to properly counsel patients.

Azoospermia, defined as the absence of sperm in the ejaculate, is a well-documented consequence of exogenous testosterone (ET) and anabolic-androgenic steroid (AAS) use. These agents suppress the hypothalamic-pituitary-gonadal (HPG) axis, leading to reduced intratesticular testosterone levels and impaired spermatogenesis. This review examines the pathophysiological mechanisms underlying azoospermia and outlines therapeutic strategies for recovery.

Tamoxifen, a common adjuvant therapy for hormone receptor-positive breast cancer, is associated with an increased risk of endometrial pathologies, such as hyperplasia, polyps, and carcinoma. This study investigates rapamycin, an mTOR inhibitor, as a potential novel strategy for preventing tamoxifen-induced endometrial proliferation. This in vitro study utilised endometrial stromal cells isolated from infertile women.

Background: In the cryopreservation process, rooster spermatozoa are vastly sensitive to reactive oxygen species (ROS). This study aimed to investigate the effects of Lake extender supplemented via Cysteamine (CYS) on the quality and fertility characteristics of rooster semen during the cryopreservation process.
Methods: Semen samples were collected from 10 proved Ross-308 roosters, diluted and cryopreserved in the Lake extender which contained 0, 1, 2, 4, and 8 mM of CYS (C-0, C-1, C-2, C-4, and C-8, respectively).

The surgical management of cancer patients wishing fertility preservation is multidisciplinary, involving surgeon, anesthetist, hematologist, and nursing and laboratory staff. Many oncology patients have a multitude of medical or surgical conditions that require careful planning of all therapy including surgical removal of reproductive material, either oocytes or ovarian tissue. The significant risks related to either transvaginal or abdominal surgery should be discussed and documented and the final decision to proceed must be balanced against the risks, including death.

Data collection from all referred patients allows continuous learning and improvement of services. Patient safety and return for cancer therapy takes priority in all oncofertility services. In certain cases, the intervention for female patients is contraindicated and, aside from extensive counseling, alternative methods of preserving fertility should be explored.
