Light curing infection control barriers: do some types jeopardize the concept of conventional bulk-fill composites?

BMC Oral Health

November 2024

Biomaterials Department, Faculty of Dentistry, Ain-Shams University, Organization of African unity, Cairo, Egypt.

Background: Using infection control barriers (ICBs) on light curing units (LCUs) became mandatory to achieve proper infection control measures without jeopardizing the integrity of the restorations, especially at deeper layers. This study explored the effect of two ICBs on the irradiance of the LCU, as well as the degree of conversion (DC) and flexural strength (FS) of two types of bulk-fill composites. Water vapor permeability (WVP) of both barriers was also assessed to evaluate the capability of such barriers to prevent transmission of blood and saliva droplets and aerosols.

Background: Uncemented acetabular components are widely used in modern total hip arthroplasty (THA). Modularity has numerous advantages including the ability to use supplementary screw fixation for the shell, and allow to switch from ceramic to polyethylene (PE) bearings and vice versa, and the use of lipped and face-changing liners.

Objective: Despite these advantages, a problem with modular PE liners is dissociation.

Introduction The idea of an uncemented, fully coated hydroxyapatite (HA) stem was introduced almost 40 years ago, aiming to achieve a solid biological fixation by preserving natural bone activity. While many studies underline the longevity of uncemented total hip replacement (THR), NHS England's Best Practice Tariff (BPT) recommends using cemented implants in patients over the age of 69, with financial penalties when this policy is not met. At the same time, the 'paradox' of increased use of uncemented implants worldwide has been well described, with many surgeons using them regardless of the age group of the patient.

Background: The advent of highly porous ingrowth surfaces and highly crosslinked polyethylene has been expected to improve implant survivorship in revision total hip arthroplasty. Therefore, we sought to evaluate the survival of several contemporary acetabular designs following revision total hip arthroplasty.

Methods: Acetabular revisions performed from 2000 to 2019 were identified from our institutional total joint registry.

Background: this study aimed to assess the long-term outcomes of (a minimum of 10-years) total hip arthroplasty with a metal-on-metal acetabular prosthesis. Methods: Eighty-nine primary total hip arthroplasties (82 patients) were performed using a Pinnacle modular metal-on-metal acetabular prosthesis. Clinical hip function outcomes were evaluated using the Japanese Orthopaedic Association hip score preoperatively and at the final follow-up.
