Nucleoside reverse transcriptase inhibitors (NRTI) require intracellular phosphorylation, which involves multiple enzymatic steps to inhibit the human immunodeficiency virus type 1 (HIV-1). NRTI-triphosphates (NRTI-TP) compete with endogenous 2'-deoxyribonucleosides-5'-triphosphates (dNTP) for incorporation by the HIV-1 reverse transcriptase (RT). Thus, a highly sensitive analytical methodology capable of quantifying at the low femtomoles/10(6) cells level was necessary to understand the intracellular metabolism and antiviral activity of NRTIs in human peripheral blood mononuclear (PBM) cells and in macrophages.

Objective: To track the complete course of the HIV epidemic among women from the city of Edinburgh who delivered babies during 1982-1995.

Methods: The performance of the modified Serodia HIV test on dried blood spots from archived neonatal metabolic screening cards stored for up to 11 years was evaluated by testing 221 cards from neonates whose mothers' HIV infection status was already known (100 HIV-positive, 121 HIV-negative). Unlinked anonymous HIV testing of cards from neonates born during 1982-1989 was then performed and the resulting prevalence data were combined with existing data from 1990-1995.

AIDS surveillance: a direct assessment of under-reporting.

Objective: To assess directly the extent of under-reporting of AIDS cases to the National AIDS surveillance system.

Design: All AIDS cases diagnosed from 1 January 1982 to 1 August 1989 in Riverside Health Authority were identified from a local register of HIV infection and cross-checked against records of AIDS cases reported to the national AIDS surveillance system.
