Although systemic lupus erythematosus is the prototype multisystem disease, many other inflammatory diseases are also promiscuous, with symptoms manifesting in many organs. Now, new data emphasize the extra-articular involvement in spondyloarthritis, predominantly affecting the eye; however, the factors that account for the observed organ involvement remain unknown.

This study aimed to explore the causal associations between 6 immune-related diseases and iridocyclitis using Mendelian randomization (MR). A total of 230 single nucleotide polymorphisms (SNPs) significantly associated with systemic lupus erythematosus, ankylosing spondylitis (AS), rheumatoid arthritis (RA), Graves disease (GD), Crohn disease (CD), and allergic contact dermatitis were identified based on stringent MR assumptions.

Retinopathy, small fiber neuropathy (SFN), and encephalopathy associated with recoverin antibodies have not been previously reported as side effects of BNT162b2 vaccination in a patient with HLA-B27-associated spondylarthritis. The patient is a 47-year-old male with a 10-year history of HLA-B27-associated spondylarthritis without recurrence, who developed acute and post-acute COVID vaccination syndrome (ACVS/PACVS) after the first dose of the BNT162b2 vaccine. The PACVS manifested as cerebral disease, eye disease, and SFN.
