Delivering radiation therapy in an oncology setting is a high-risk process where system failures are more likely to occur because of increasing utilization, complexity, and sophistication of the equipment and related processes. Healthcare failure mode and effect analysis (FMEA) is a method used to proactively detect risks to the patient in a particular healthcare process and correct potential errors before adverse events occur. FMEA is a systematic, multidisciplinary team-based approach to error prevention and enhancing patient safety. We describe our experience of using FMEA as a prospective risk-management technique in radiation oncology at a national network of oncology hospitals in the United States, capitalizing not only on the use of a team-based tool but also creating momentum across a network of collaborative facilities seeking to learn from and share best practices with each other. The major steps of our analysis across 4 sites and collectively were: choosing the process and subprocesses to be studied, assembling a multidisciplinary team at each site responsible for conducting the hazard analysis, and developing and implementing actions related to our findings. We identified 5 areas of performance improvement for which risk-reducing actions were successfully implemented across our enterprise.

Background: There is an under-reporting of anaesthesia-related safety events. Incident-capturing systems (ICSs) are essential for patient safety monitoring, identifying risks and ongoing opportunities for improvement. After a literature review and assessment of our current ICSs, we concluded that our institution lacked a reliable anaesthesia-specific ICS system, leading to under-reporting of anaesthesia-related safety events.

Objectives: To compare the mechanical performance of partially replaced (repaired) intra-coronal restorations to totally replaced ones in root canal-treated teeth.
Methods: Thirty maxillary second premolars were selected according to strict criteria, mounted on moulds, and had mesio-occluso-distal (MOD) cavities prepared. Resin composite restorative material was used to perform the initial restoration, followed by aging procedures using thermo-mechanical cycling fatigue to replicate six months of intraoral aging.

Metal and polymer interface bonding significantly influences the mechanical performance of fiber metal laminates (FMLs). Therefore, the effect of surface treatments (mechanical abrasion, nitric acid etching, P2 etching, sulfuric acid anodizing (SAA), and electric discharge machine (EDM) texturing) carried on aluminum 2024-T3 alloy sheets was evaluated considering surface morphology, surface topography, and surface roughness. Further, the influence of surface treatments on interfacial adhesion strength and failure mode between the aluminum alloy and carbon fiber prepreg was investigated.

To solve the problems of insufficient stiffness and poor integrity of traditional F-type socket joints, steel screw connections are set along the longitudinal direction between rectangular pipe jacking joints. However, the mechanical properties of F-type socket joints with steel screw connections have not been fully investigated, and the influence of the coefficient of subgrade reaction has not been considered. In this work, through model tests and numerical simulations of F-type socket joints with steel screws under different coefficients of subgrade reaction, the influence of steel screws on the deformation and damage characteristics of F-type socket joints is discussed, and the bending mechanical response of F-type socket joints under different coefficients of subgrade reaction is analyzed.

The existing calculation method for the surrounding rock pressure of shallow buried bias tunnel fails to account for the impact of the progressive failure characteristics of the surrounding rock and slope creep, thereby neglecting the additional pressure arising from slope creep. Therefore, the progressive instability failure mode of the surrounding rock of shallow buried bias tunnel was obtained by numerical simulation. Based on this, the theoretical analysis model of the additional pressure of shallow buried bias tunnel was established, and the calculation formula of the additional pressure was derived.
