Hypothesis: Mechanical stimulation through a cochlear third window into the scala tympani in a chinchilla model with normal and fixed stapes can generate cochlear responses equivalent to acoustic stimuli.
Background: Cochlear stimulation via the round window (RW) using active middle ear implants (AMEIs) can produce physiologic responses similar to acoustic stimulation including in a model of stapes fixation. However, pathologic conditions, such as advanced otosclerosis, can preclude delivery of sound energy to the cochlea through the oval window and/or the RW.
Methods: Cochlear microphonic (CM) and laser Doppler vibrometer measurements of stapes and RW velocities were performed in 6 ears of 4 chinchillas. Baseline measurements to acoustic sinusoidal stimuli (0.25-8 kHz) were made. Measurements were repeated with an AMEI driving the RW or a third window to the scala tympani before and after stapes fixation.
Results: AMEI stimulation of the third window produced CM waveforms with morphologies similar to acoustic stimuli. CM thresholds with RW and third-window stimulation were frequency dependent but ranged from 0.25 to 10 and 0.5 to 40 mV, respectively. Stapes fixation, confirmed by laser Doppler vibrometer measurements, resulted in a significant frequency dependent impairment in CM thresholds up to 13 dB (at <3 kHz) for RW stimulation and a nonsignificant frequency-dependent improvement of up to 10 dB (at >3 kHz) via third-window stimulation.
Conclusion: AMEI mechanical stimulation through a third window into the scala tympani produces physiologic responses nearly identical to acoustic stimulation including in a model of stapes fixation with decreased efficiency.

Objective: Although NOG variants are linked to congenital stapes fixation and conductive hearing loss (CHL), little is known about middle ear surgery outcomes and the characteristics of accompanying inner ear anomalies. We explored auditory phenotypes in patients with NOG variants, with a focus on the outcomes of middle ear surgery.
Methods: This study included 11 patients from five unrelated Korean families harboring NOG variants.

Otosclerosis leads to stapes fixation and consequent conductive hearing loss. Surgery is the mainstay of treatment, and it can be achieved through small fenestra stapedotomy or stapedectomy. Despite the first being favored by most, evidence supporting its superiority over the latter remains inconclusive.

Aim: To evaluate the audiometric results of tympanoplasty with stapedectomy and insertion of a synthetic total ossicular replacement prosthesis (TORP).
Material And Methods: Retrospective observational study conducted on a cohort of 15 patients (16 ears) aged 10 to 58 years (mean: 36 years) with chronic otitis media (COM) and tympanosclerosis (n=9), stapediovestibular ankylosis (n=3), minor aplasia (n=3) or post-traumatic ossicular dislocation (n=1). Treatment consisted in tympanoplasty with stapedectomy and TORP placement by the same operator, between December 1, 2012 and January 30, 2023, in a French university hospital department, with follow-up ranging from 2 to 92 months (mean: 24 months).

Retrospective analysis of clinical data of 123 patients with atticotomy, exploring the clinical characteristics of patients undergoing atticotomy and the efficacy of hearing reconstruction methods. 123 patients with atticotomy were divided into three groups according to the ossicular chain treatment method: preservation of the ossicular chain group(37 cases), cartilage elevation of stapes group(49 cases), and PORP group(37 cases). The clinical characteristics of patients with atticotomy, preoperative and postoperative hearing levels of the three groups of patients, and postoperative complications were analyzed.
