Patients with bronchopleural fistula are at an increased risk of morbidity and mortality. Treatment of the air leak includes intrathoracic drainage, antibiotic therapy and closure of the fistula, which conventionally has been performed via surgical means. In patients with limited respiratory capacity, less-invasive alternatives are required. Here we report on a 62-year-old patient with underlying severe COPD, who was admitted with a lung abscess and consecutively developed a persistent bronchopleural fistula. Treatment involved antibiotic therapy and endobronchial one-way valve placement, which resulted in termination of the air leak and full recovery.

Persistent air leak following a pneumothorax refers to air leakage lasting 5-7 days after the initial event. Different strategies have been used with varying degrees of success including surgical or chemical pleurodesis. Endobronchial valve (EBV) insertion is a technique where the insertion of a one-way valve obstructs the flow of air through the leak and helps in pneumothorax resolution especially if surgery is contraindicated.

Multiple distinct devices and technologies, including endobronchial coils, thermal vapor ablation, bio-lung volume reduction, and airway bypass stenting, have been developed to achieve lung volume reduction with varying degrees of accessibility and evidence.
