Department of Obstetrics and Gynaecology, University of British Columbia, and the Children's and Women's Hospital of British Columbia, Vancouver, British Columbia, Canada.

Published: March 2012

Objective: To examine temporal trends in clomiphene citrate use and sociodemographic characteristics associated with treatment initiation and duration.

Design: Retrospective cohort study.

Setting: British Columbia, Canada, 1996 to 2008.

Patient(s): Women aged 20 to 49 years.

Intervention(s): None.

Main Outcome Measure(s): Temporal trends in treatment prevalence and incidence rates were assessed using Cochran-Armitage test for trend; differences in treatment duration were assessed using Wilcoxon and Kruskal-Wallis tests. Incidence rate ratios and 95% confidence intervals were estimated using Poisson regression.

Result(s): A province-wide database of all dispensed prescriptions was used to identify clomiphene citrate users. Rural versus urban residence and neighborhood's socioeconomic quintile were identified based on postal codes. Overall, 26,993 women were prescribed clomiphene citrate between 1996 and 2008. Prevalence rates declined from 4.1 to 3.7 clomiphene users per 1,000 women during this period, while the incidence rates remained stable (2.2 new users per 1,000 women per year). Both prevalence and incidence rates declined among women aged 20 to 29 years, and showed a statistically significant increase among women aged 30 to 44 years. Women aged 30 to 34 years had the highest incidence of clomiphene use (4.5 per 1,000 women per year). Urban residence and low versus high socioeconomic status were associated with higher rates of treatment initiation. Older age (40 to 49 years) and urban residence were associated with shorter duration of use.

Conclusion(s): The increase in the incidence rates of clomiphene citrate use among women aged 30 to 44 years indicates a growing demand for fertility treatments in this population and may have contributed to the increase in multiple births.

Objective: To explore whether acupuncture combined with clomiphene can reduce the luteinizing hormone-to follicle-stimulating hormone ratio and impact the gut microbiota in patients with obese polycystic ovary syndrome.

Methods: This open-label, randomized, parallel-group controlled trial included 86 women aged 20-40 years with obese polycystic ovary syndrome and 19 healthy controls. Participants were randomly assigned to either an acupuncture combined with clomiphene group or a clomiphene-only group, with a healthy control group for comparison.

Purpose: To evaluate the effect of transvaginal ovarian drilling (TVOD) on IVF outcomes in subjects with clomiphene-resistant PCOS and a history of IVF failure.

Methods: Between 2008 and 2011, 19 subjects with sonographically PCOS and a history of failure to ovulate to high-dose clomiphene citrate were prospectively followed and underwent TVOD at a university hospital-based IVF program.

Results: In 15 subjects who underwent 30 fresh paired IVF cycles TVOD resulted in a significantly higher number of oocytes retrieved (7.

Purpose: Clinical trials are advancing the treatment of polycystic ovary syndrome (PCOS), an endocrine disorder affecting 8-13% of women. Lifestyle interventions, including nutritional plans, physical activity, and stress management, can improve reproductive hormones and metabolic health. Novel pharmacotherapies targeting hormonal, metabolic, and reproductive abnormalities are being explored for individualized treatment.

The selective estrogen receptor modulator clomiphene inhibits sterol biosynthesis in Arabidopsis thaliana.

Sterols are produced via complex, multistep biosynthetic pathways involving similar enzymatic conversions in plants, animals and fungi, yielding a variety of sterol metabolites with slightly different chemical properties to exert diverse and specific functions. A tremendously diverse landscape of sterols, and sterol-derived compounds, can be found across the plant kingdom, determining a wide spectrum of functions. Resolving the underlying biosynthetic pathways is thus instrumental to understanding the function and use of these molecules.
