Skin tags, medically known as acrochordons, are harmless growths of the epidermis that commonly develop in areas where the skin folds, such as the neck, armpits, or groin. While usually asymptomatic, these lesions can cause discomfort from rubbing or cosmetic issues. They are more prevalent in middle-aged and older individuals and are often correlated with conditions such as obesity, diabetes, and insulin resistance.

A groin mass can arise from multiple conditions, with common causes including hernias, enlarged lymph nodes, fluid collections, or tumors. An ultrasound, together with a clinical examination, plays a key role in diagnosing groin masses and identifying hidden hernias. A 44-year-old male presented with a painless right inguinal mass that had been continuously present for one month.

Background: Extraskeletal mesenchymal chondrosarcoma (ESMC), an uncommon and highly aggressive form of chondrosarcoma, is characterized by its mesenchymal origin and absence of skeletal involvement. Only a few cases of primary ESMC with metastasis to the pancreas have been reported so far. In this study, we present a case of ESMC in the left thigh with a solitary pancreatic metastasis in a 45-year-old woman.
