According to WHO estimates, between 1 and 20% of tuberculosis cases in the world are multiresistant. In Gabon, this prevalence is estimated at 1.9%. In this forward-looking study from March 2006 to August 2010, we report 16 cases of multi-resistant tuberculosis out of 24 suspected resistant samples (persistence of the clinical and radiological signs after three months of well conducted treatment with first-line anti-tuberculous drugs). This study is realized in association with the laboratory of mycobacterium of the Percy military teaching hospital, Clamart, France.

Background: Fluoroquinolones are used for the complex treatment of mono-, poly-, and multi-resistant Tuberculosis (TB) and are the most efficient among resistant TB treatment drugs. Disease caused by fluoroquinolone-resistant TB strains, especially pre-extensive TB (pre-XDR) and extensive (XDR) forms are extremely hard to manage, and treatment efficacy is quite low. With the revitalization and extension of resistant TB drugs, one of the main research domains is to study resistance inhibitors aimed at restoring the efficacy of main and priority anti-TB medications.

Linezolid combined with rifampicin has shown excellent clinical outcomes against infection by multi-resistant Gram-positive bacteria. However, several studies have indicated that rifampicin reduces the plasma concentration of linezolid in patients with severe infection. Linezolid has been recommended for the treatment of patients with multidrug-resistant or extensively drug-resistant tuberculosis.

Tuberculosis (TB) is a leading cause of death worldwide. TB represents a serious public health threat, and it is characterized by high transmission rates, prevalence in impoverished regions, and high co-infection rates with HIV. Moreover, the serious side effects of long-term treatment that decrease patient adherence, and the emergence of multi-resistant strains of Mycobacterium tuberculosis, the causing agent of TBs, pose several challenges for its eradication.
