AI Article Synopsis

  • The case discusses a 72-year-old woman with both Cogan-Reese syndrome and exfoliation syndrome impacting her right eye, resulting in high intraocular pressure (IOP) and severe visual loss.
  • The left eye remains unaffected, highlighting the unusual nature of having both syndromes coexist in a single eye leading to secondary open-angle glaucoma.
  • This case is significant as it's the first reported instance of these two conditions occurring together, suggesting that both may contribute to the development of glaucoma.

Article Abstract

Purpose: To present a case with co-existence of Cogan-Reese syndrome and exfoliation syndrome.

Case Report: A 72-year-old Caucasian woman presented for consultation due to uncontrolled intraocular pressure (IOP) of the right eye. Clinical examination revealed the presence of Cogan-Reese syndrome and exfoliation syndrome OD. This eye exhibited elevated IOP, open anterior chamber angle, advanced glaucomatous optic nerve head damage, and severe glaucomatous visual field loss. The left eye was completely normal without IOP elevation or visual field damage. To our knowledge this is the first case report demonstrating the coexistence of the Cogan-Reese syndrome, exfoliation syndrome, and secondary open-angle glaucoma. Since both syndromes frequently lead to secondary open-angle glaucoma, it is not feasible to determine with certainty which condition was the cause of secondary open-angle glaucoma in our case. It is conceivable that both conditions contributed to glaucoma development.

Conclusions: Cogan-Reese syndrome, exfoliation syndrome and secondary open-angle glaucoma may present on the same eye.

Background: Iridocorneal endothelial (ICE) syndrome is a group of ophthalmic disorders, first reported by Eagle and Yanoff in 1979, a disease characterized by abnormalities of the iris and the corneal endothelium, and mainly occurs unilaterally in young and middle-aged women, with no family history. ICE syndrome comprises a spectrum of three clinical variants: Progressive essential iris atrophy (corectopia, iris atrophy or iris hole), Chandler syndrome (corneal oedema with mild to absent iris change), and Cogan - Reese syndrome (nodular pigmented lesion of the iris).

Objective: We are presenting this case because of its rarity, diagnostic intricacy and therapeutic challenge.

Department of Ophthalmology, Gavin Herbert Eye Institute, University of California, Irvine, California, USA.

The aim of this study was to report a novel presentation of Cogan-Reese syndrome presenting with zonular dehiscence during cataract extraction with intraocular lens placement. A 49-year-old woman presented with worsening vision over 2 years. The examination was significant for bilateral pupil miosis, visually significant cataracts, and unilateral glaucoma.
