Abdominal approach is commonly used for resection of liver tumors. However, in rare cases, transthoracic approach may be a valuable option for management of lesions located in the hepatic dome or involving the cavo-hepatic junction for very selected patients. This approach can be an open procedure (thoracotomomy), a video-assisted minimally invasive technique (thoracoscopy), or a strictly percutaneously treatment (CT-guided radiofrequency ablation). This approach seems useful for high-risk patients, with previous major abdominal surgery, or awaiting for liver transplantation (bridge concept) with cranially located single lesions. A limited liver resection (tumorectomy or segmentectomy) can be performed, but this approach is also suitable for percutaneous ablation therapy (radiofrequency or cryotherapy), with an acceptable morbidity.

Background: Although the prognosis in systemic lupus erythematosus (SLE) has dramatically improved, pulmonary artery hypertension (PAH) is one of the life-threatening comorbidities associated with SLE. The management of the comorbidity is occasionally challenging due to the lack of consensus regarding treatment options including immunosuppressive agents, selective pulmonary vasodilators, and cardiac rehabilitation.
Case Summary: A 28-year-old female who terminated prednisolone after remission of SLE by her own discretion 3 years ago developed dyspnoea on effort.

The study was designed to investigate the pattern of intraventricular Hemo-Dynamic Forces (HDF) and myocardial performance during exercise in Elite Cyclists (EC). Transthoracic stress echocardiography was performed on nineteen EC and thirteen age-matched sedentary controls (SC) at three incremental exercise intensities based on Heart Rate Reserve (HRR). Left Ventricular (LV) HDF were computed from echocardiography long-axis data sets using a novel technique based on endocardial boundary tracking, both in apex-base and latero-septal directions.
