Transient osteoporosis of the hip (TOH) is a rare condition mainly affecting pregnant women in their third trimester and middle aged men. We report a case of TOH/Bone marrow edema syndrome in pregnancy with involvement of the surrounding soft tissues on magnetic resonance image, which has not been previously reported. The presence of such edema in the soft tissues may help to differentiate this condition from early avascular necrosis of the hip, and may also provide an insight into the pathogenesis of the condition. The reported patient was treated conservatively and fully recovered.

Atraumatic bilateral femoral neck fractures are rare, especially in younger patients and those without significant comorbidities. However, pregnant individuals appear to be at increased risk due to normal physiological changes in calcium balance, leading to transient osteoporosis of the hip. In these individuals, calcium and bone mineral density are generally decreased, reflecting the calcium demands of the developing fetus.

Transient osteoporosis of the hip is an uncommon bone marrow oedema syndrome of unknown pathophysiology affecting previously healthy pregnant women and young to middle-aged men. The disorder is characterised by hip pain, reduced mobility, and the risk of fracture, which is significantly higher in pregnant than in non-pregnant individuals. Diagnosis is by magnetic resonance imaging.

Introduction: Transient osteoporosis of the hip (TOH) presents with symptoms such as groin pain, discomfort, and the presence of bone edema detected on magnetic resonance imaging (MRI) scans. We have observed an increase in the occurrence of TOH in patients following the COVID-19 pandemic.
Case Report: In this series, we present four cases of TOH that developed after COVID-19 infection.
