Objective: Heart-focused anxiety is a common phenomenon that is related to psychological disorders and cardiac diseases. We investigated heart-focused anxiety in the general population and provided standard values using the Cardiac Anxiety Questionnaire (CAQ). Furthermore, we assessed the reliability of the CAQ and investigated the influence of social variables on heart-focused anxiety.

Methods And Results: The questionnaire was applied to 2,396 individuals (age range: 18-92; mean age 49 ± 17, 50% male). Three aspects of heart-focused anxiety (fear, attention, and avoidance) were captured by the CAQ. To test the influence of age, gender, and social factors, we performed analyses of variance, correlative statistics, and regression analyses. Heart-focused anxiety showed a linear increase with age (P < 0.001). No gender differences were observed. The degree of anxiety was affected by relationship (P < 0.001), level of education (P < 0.001), employment (P < 0.001), and income (P < 0.001).

Conclusion: We evaluated heart-focused anxiety in the general population and validated the CAQ. Using percentiles based on a normative sample allows screening for heart-focused anxiety in patients with or without cardiac diseases and the measurement of therapy success after psychotherapeutic or pharmacological intervention.

Article Synopsis
  • Heart-focused anxiety is the fear and concern about heart-related issues and their serious consequences, leading to avoidance behaviors and frequent medical help-seeking.
  • A study was conducted with 120 patients before cardiac catheterization to assess how Emotional-Focused Coping techniques impacted their anxiety levels, using a pretest and posttest design.
  • Results showed a significant reduction in anxiety levels after applying these coping strategies, indicating they are effective and easy interventions for managing heart-focused anxiety.

Associations of fear of physical activity, coping style and self-reported exercise behavior in patients with chronic heart failure.

PLoS One

September 2024

Department of Internal Medicine III, Cardiology, Angiology and Intensive Care Medicine, University Hospital, Saarland University, Homburg Saar, Germany.

Article Synopsis
  • The study investigates how fear of physical activity (PA) impacts exercise habits in heart failure (HF) patients, linking it to heart-focused anxiety and trait anxiety.
  • Conducted with 185 HF outpatients, results show that higher NYHA class and more HF medications are linked to greater fear of PA, which in turn leads to avoidance of physical activities and lower participation rates in sports/exercise.
  • It concludes that patients who are more attentive and distressed by their HF symptoms tend to have a higher fear of PA, especially those with a vigilant coping style that focuses on health threats.

Department of Congenital Heart Disease and Pediatric Cardiology, Deutsches Herzzentrum München, Technische Universität München, München, Germany (Ms Willinger and Drs Hock, Hager, Oberhoffer-Fritz, Ewert, and Müller); Institute of Preventive Pediatrics, Technische Universität München, München, Germany (Ms Willinger and Drs Oberhoffer-Fritz and Müller); and DZHK (German Centre for Cardiovascular Research), Partner Site Munich Heart Alliance, Munich, Germany (Dr Ewert).

Purpose: Psychological distress and anxiety are increased among adults with congenital heart disease (CHD). However, heart-focused anxiety (HFA) and its impact on peak oxygen uptake (V˙ o2peak ) have not yet been studied in this patient cohort. This study investigates HFA in adults with CHD and whether HFA is associated with V˙ o2peak in these patients.
