Background/rationale: There is an increase of spontaneous consultation and a need for effective management of it. For this reason we created the "High Resolution Nursing Consultation" (CIAR), giving nurses greater autonomy.
Objectives: We aim to demonstrate the ability of nursing in the resolution of spontaneous consultation as well as to know demands more frequent and what is more resolving and to find out the correlation between the reason for visit registered at admissions and real.
Description: Cross sectional study In CIAR, a primary care nurse serves adult patients who requested spontaneous consultation, without any preselection. We analyze results of eight months of consultation. The visits were performed according to protocols developed in the center.
Results: Nursing resolved 63% of them, 37% jointly Independent resolution increased to 75.7% in the last month. The most common reasons were: administrative, respiratory disease and otolaryngology. Nursing decided autonomously 98% of administrative visits, 80% of wounds/burns, 72% of the toothache and between 50% and 60% of GYN/emergency contraception, ophthalmology, internal medicine and digestive. With the implementation of the protocol of acute nasopharyngitis nursing resolution increased from 4.9% to 78.3%., the correlation between the reason for visit registered at admissions and real was 96%.
Conclusions: Nursing is highly qualified for the management of spontaneous consultation with excellent results. Protocols are of great help in the resolution of nursing and it improves with the experience acquired.

T-regulatory-type-1 (TR1) cells are a subset of interleukin-10-producing but Foxp3 Treg cells that arise in response to chronic antigenic stimulation. We have shown that systemic delivery of autoimmune disease-relevant peptide-major histocompatibility complex class II (pMHCII)-coated nanoparticles (pMHCII-NP) triggers the formation of large pools of disease-suppressing Foxp3 TR1 cells from cognate T-follicular helper (TFH) cell precursors. Here we show that, upon treatment withdrawal, these Foxp3 TR1 cells spontaneously differentiate into a novel immunoregulatory Foxp3 TR1 subset that inherits epigenetic and transcriptional hallmarks of their precursors, including clonotypic T-cell receptors, and is distinct from other Foxp3 Treg subsets.

We present a case of symptomatic PLE with annular contrast enhancement on MRI suggestive of an infectious cause. We show that careful clinical evaluation and laboratory testing, in addition to neuroimaging, are essential to guide treatment and to avoid unnecessary interventions in PLE cases with a favorable spontaneous course.

Objectives: To assess the effectiveness and anatomical advantages of the lateral transorbital approach for complex skull base repairs in the lateral recess of a pneumatized sphenoid sinus and compare it to standard transnasal methods.
Methods: Case series of four patients across two institutions (Otorhinolaryngology and Neurosurgery departments in Cape Town, South Africa, and Seattle, USA). Three patients with spontaneous cerebrospinal fluid (CSF) leaks underwent a pure lateral transorbital repair; one patient with skull base fractures from a gunshot wound received a combined transnasal and transorbital repair with a nasoseptal flap through the lateral transorbital corridor.

Introduction: A previous study found that following out-of-hospital cardiac arrest (OHCA), 67% of out-of-hospital 12-lead electrocardiograms (ECGs) diagnostic for ST-segment elevation myocardial infarction (STEMI) changed to non-STEMI on repeat emergency department (ED) ECG. Here we evaluated associations with resolution of STEMI on ED ECG.
Methods: In this secondary analysis of a previous retrospective study, adults (≥18 years) with return of spontaneous circulation (ROSC) following OHCA, at least 1 out-of-hospital and ED ECG and transport to the study hospital were entered.
