Penetrating neck trauma: a 5-year review of the literature, 1983 to 1988.

Penetrating neck trauma remains controversial: some trauma centers continue to pursue a policy of mandatory exploration while others advocate selective exploration. The literature regarding penetrating neck trauma published during the past 5 years is reviewed in this report. The majority of reports support selective exploration, and most civilian centers report a mortality of 3% to 6% regardless of the type of exploration performed. To clarify the rationale behind the selective management of penetrating neck wounds, current data on ballistics, ancillary diagnostic studies, and comparative costs are reviewed. Emergency room management and surgical follow-up, which vary according to the type of missile and the zone of the neck penetrated, are discussed.

 There are no clear guidelines for deciding between endoscopic sinus surgery and tooth extraction for the treatment of odontogenic sinusitis. Furthermore, tooth extraction does not necessarily improve sinusitis and eventually results in additional endoscopic sinus surgery.  The present study aimed to retrospectively investigate negative predictive factors of tooth extraction for odontogenic sinusitis.

We report a case of a 45-year-old gentleman who presented to our major trauma centre after sustaining a penetrating high-pressure paint injection injury to the neck. This rare mechanism of injury is most commonly reported to affect the non-dominant hand, occurring due to the malfunction or misuse of industrial paint machines, causing a piercing soft tissue injury with high-pressure fluid. The unique challenges faced in managing penetrating injuries to the neck are due to the density of vital visceral structures in the region, including major blood vessels and the upper aerodigestive tract.
