[Finding the drug in cutaneous drug eruptions].

Rev Med Liege

Service de Dermatopathologie, CHU de Liège, Belgique.

Published: September 2011

Drug eruptions are frequently encountered. Their putative diagnosis is based on a set of imputability arguments. The histopathological aspect is often evocative of the nature of the dermatosis. It varies according to the type of drug reaction. Some drug eruptions follow a benign course, but others are life-threatening.

A 53-year-old woman undergoing combination therapy with epidermal growth factor receptor (EGFR) and vascular endothelial growth factor receptor (VEGFR) inhibitors for advanced lung cancer with brain metastases developed pustules and punctate purpura on both lower extremities. Histopathological examination revealed neutrophilic infiltration around the hair follicles and erythrocyte extravasation in the perivascular regions near the hair roots, leading to a diagnosis of purpuric papulopustular eruptions. The rash improved with oral doxycycline (100 mg/day) and topical corticosteroids.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome and haemophagocytic lymphohistiocytosis (HLH) are rare but severe immune-mediated diseases with overlapping clinical manifestations. We present a case of a woman in her late 40s with rheumatoid arthritis who developed DRESS/HLH overlap syndrome after starting hydroxychloroquine and leflunomide therapy. Despite corticosteroid treatment, her condition worsened, necessitating etoposide therapy.

Background: Toxic epidermal necrolysis (TEN), a severe cutaneous hypersensitivity reaction induced particularly by drugs, is diagnosed when there is a fever of ≥ 38 °C, mucocutaneous symptoms, a rash with multiple erythema, and skin peeling of ≥ 30% of the body surface area. The mortality rate of TEN is high, and thrombocytopenia during treatment can lead to severe outcomes. Intravenous immunoglobulin (IVIg) is used when steroids are ineffective in TEN and may improve mortality; however, thrombocytopenia is a rare adverse event associated with IVIg use.

We present a case of a female patient in her 30s with acute generalised exanthematous pustulosis following vancomycin treatment for left breast cellulitis.
