Publication of the treat-to-target (T2T) concept at the end of the Bone and Joint Decade almost coincided with the 15 treatment recommendations of the EULAR as well as with the new classification and revised remission criteria for rheumatoid arthritis (RA).The treat-to-target initiative is solely focused on the description of benchmarks for an effective treatment management of RA to reach its priority target of remission. From the perspective of inpatient rheumatology parallels are shown to the quality projects of the Association of Rheumatology Clinics (VRA), which were initiated in 2001 for the first time. The quality management started up by the VRA not only benefited from the development of the German diagnosis-related groups (G-DRG) system which was also supported by the VRA and the German Society of Rheumatology (DGRh) but also from projects realized to improve the structural and operational quality in inpatient rheumatology units.By launching its own outcome benchmarking project (OBRA) in 2003, which was financially supported by the German Ministry of Health and its continuation with Kobra-1 and future Kobra-2, the VRA already successfully incorporated all benchmarks of management guidelines outlined in the T2T initiative.By steadily improving the outcome, even of RA patients, inpatient rheumatology offers a special quality project for participating clinics who are otherwise competitors, not only to learn from each other but to support each other to find the best way (T2T) to reach a remission and a good quality of life for RA patients and patients with other systemic rheumatic diseases.

Patients with diabetes (DM) are at an increased risk of infection, with urinary tract infections (UTIs) being common among individuals with type 2 DM (T2D). The aim of this study was to determine the prevalence and risk factors for UTIs among hospitalized T2D patients from Timișoara, Romania. The hospital records of 1139 T2D adult inpatients who were ordered to provide urine cultures during hospitalization were reviewed.

Introduction: Sciatica is a debilitating condition that often becomes chronic, and for which there are few effective treatment options. Treatments such as the anti-depressant duloxetine have shown promise, but the evidence is inconclusive. We are describing a high quality, definitive trial to investigate the efficacy, safety and cost-effectiveness of duloxetine in chronic sciatica.

Objective: The study investigates long-term outcomes of unselected inpatients undergoing invasive coronary angiography (CA) with and without diabetes mellitus type II (T2DM).
Background: Due to continual shifts in demographics and advancements in treating cardiovascular disease, there has been a notable evolution in the types of patients undergoing CA over the past decades. Comprehensive data on the extended outcomes of CA patients, both with and without concurrent T2DM, remains scarce.
Background: Diarrhoeal diseases claim more than 1 million lives annually and are a leading cause of death in children younger than 5 years. Comprehensive global estimates of the diarrhoeal disease burden for specific age groups of children younger than 5 years are scarce, and the burden in children older than 5 years and in adults is also understudied. We used results from the Global Burden of Diseases, Injuries, and Risk Factors Study 2021 to assess the burden of, and trends in, diarrhoeal diseases overall and attributable to 13 pathogens, as well as the contributions of associated risk factors, in children and adults in 204 countries and territories from 1990 to 2021.

Objective: To test the ability of the 2019 EULAR/ACR Classification Criteria for SLE score to predict lupus related hospitalization and overall cost of hospitalization.
Methods: 217 University of Kentucky patient records that met our preliminary inclusion criteria, 44 patients were selected by a random number generator algorithm for a thorough chart review to collect data needed for calculation of the 2019 EULAR/ACR Classification Criteria for SLE score. Total hospitalization cost was calculated by using hospital adjusted expenses per inpatient day data, which estimates the expense incurred by the hospital to provide services and thus removes the variability of charges and reimbursements introduced by insurance type.
