Thymoma-associated multiorgan autoimmunity (TAMA) is a rare paraneoplastic disorder that presents similarly to graft versus host disease (GVHD). We report a unique case of TAMA presenting as a GVHD-like erythroderma in an elderly male with a history of benign thymoma. Cutaneous histopathological findings demonstrated vacuolar interface dermatitis with numerous dyskeratotic keratinocytes, exocytosis of lymphocytes, and a mildly acanthotic epidermis, which can be seen in several different disease processes.

Cutaneous paraneoplastic syndromes may be present at the time of lymphoma diagnosis, whereas in other patients, it may appear at the time of relapse, including patients with initial absence of cutaneous manifestations during the initial lymphoma presentation.

Ichthyosiform eruption of mycosis fungoides is rare, and ichthyosis with superimposed mycosis fungoides is scarcely ever seen; only a few cases have been documented in the medical literature. Furthermore, the patients with ichthyosiform lesions may indicate the presence of an underlying systemic disease, paraneoplastic syndrome, or an atypical manifestation of cutaneous T-cell lymphoma. Thus, determining the accurate etiology is important to establish the correct diagnosis and subsequently facilitate its management.
