Case Presentation: A 30-year-old man with a history of treated pulmonary embolism had progressive dyspnea. Tests revealed a mass in the right atrium and a new pulmonary embolism.

Valvular heart disease, and in particular, rheumatic mitral stenosis is frequently associated with intra-cardiac thrombus. Moreover, almost all types of thrombus can be founded in the left atrium whereas the ball shaped thrombus remains very rare. The following case report describes a successful surgical management of an unusual case with a concomitant left atrial free-floating ball thrombus and mural one in a patient who has rheumatic mitral stenosis with atrial fibrillation.

Intracardiac thrombi are rare in dogs. If they occur, they are mostly seen in the right atrium while only two case reports describing a mural left atrial thrombus in dogs are available. This case report describes a 14-year-old mixed-breed bitch that was presented at the clinic because of dyspnoea.

Background: Uterine leiomyomas represent the gynecological neoplasm with the highest prevalence worldwide. This apparently benign pathological entity may permeate into the venous system causing the so-called intravenous leiomyomatosis of the uterus (IVL). IVL may seldom extend to large caliber veins and reach the right cardiac chambers or pulmonary arteries and cause signs of right sided congestive heart failure and sudden death.

What Is Known And Objective: Free-floating thrombi in the right heart occur in a subset of patients with pulmonary embolism with high rates of mortality if untreated. Treatment of choice is controversial and likely patient specific as studies have failed to consistently show a significant difference in mortality between options.

Case Summary: We present a case of a 45-year-old female with a mobile right heart thrombus treated with ultra-slow, low-dose t-PA infusion in combination with heparin.
