Normal ageing of the cardiopulmonary system overlaps with the development of cardiovascular disease. It is characterised by changes which decrease cardiovascular reserve through senescent changes in myocardial function and volumes. These changes are compounded by co-existing cardiovascular disease, multisystem co-morbidities and polypharmacy. Indices of vascular ageing permit risk prediction in the perioperative setting. The investigation of these changes using arterio-ventricular coupling has highlighted preload sensitivity and beta-adrenergic and baroreflex insensitivity as the cardinal features which influence cardiovascular performance. Anaesthesia interferes with each of the components and poses substantial challenges. Comprehensive preoperative evaluation and optimisation assist in choosing an individualised approach. This approach, outlined here, must aim where possible to maintain tight haemodynamic control and to limit exhaustion of cardiopulmonary reserve.

Changes in the microstructure of the aortic wall precede the progression of various aortic pathologies, including aneurysms and dissection. Current clinical decisions with regards to surgical planning and/or radiological intervention are guided by geometric features, such as aortic diameter, since clinical imaging lacks tissue microstructural information. The aim of this proof-of-concept work is to investigate a non-invasive imaging method, diffusion tensor imaging (DTI), in ex vivo aortic tissue to gain insights into the microstructure.

Background: To investigate the relevance of hyperperfusion on computerised perfusion imaging (CTP) in the emergency setting in people with non-convulsive status epilepticus (NCSE) and previous stroke, to derive relevant aspects on the epileptogenic focus and the network recruited for NCSE propagation.
Methods: We enrolled consecutive adult patients with acute-onset NCSE and a previous stroke at a single institution undergoing CTP and EEG during symptoms. All patients underwent standard imaging including CT, CTP, CT angiograms and standard EEG within 30 min from hospital arrival.
