Hypertrophic cardiomyopathy (HCM) is a complex and heterogeneous cardiac disorder, often complicated by cardiogenic shock, a life-threatening condition marked by severe cardiac output failure. Managing cardiogenic shock in HCM patients presents unique challenges due to the distinct pathophysiology of the disease, which includes dynamic left ventricular outflow tract obstruction, diastolic dysfunction, and myocardial ischemia. This review discusses current and emerging therapeutic strategies tailored to address the complexities of HCM-associated cardiogenic shock and other diseases with similar pathophysiology that provoke left ventricular outflow tract obstruction.

ECMO for bridging lung transplantation.

Eur J Med Res

December 2024

Department of Critical Care Medicine, Shulan Hangzhou Hospital, Shulan International Medical College, Zhejiang Shuren University, Hangzhou, 310022, People's Republic of China.

Background: With the shift in donor lung allocation from blood type and waiting order to the use of the lung allocation score (LAS) system, there are increasingly more cases of ECMO bridging lung transplantation. However, there are still some problems in case selection, implementation, and management.

Methods: We analyzed and summarized a series of data on ECMO bridging lung transplantation through an extensive literature review.

Background: Long segmental congenital tracheal and tracheobronchial stenosis are a rare congenital airway anomaly with variable arborizations. This study aims to analyze presentations and outcomes of slide- tracheoplasty in long segmental congenital tracheal and tracheobronchial stenosis with variable arborizations METHODS: Retrospective analysis of all patients underwent slide tracheoplasty between March 1995 to Feb 2023 for long segmental congenital tracheal and tracheobronchial stenosis. Preoperative airway morphology was divided into anatomic types based on the Great Ormond Street Children Hospital Morphological Classification.

Background: Donor stagnation and modification of lung allocation scores has resulted in a higher acuity of patient presentation prior to lung transplantation. Extracorporeal membrane oxygenation (ECMO) has been utilized as a bridge to lung transplant (BTT) although the effect of cannulation strategy on outcomes has not been well investigated. We sought to analyze contemporary data on ECMO BTT utilizing a large, international registry of patients.

Patients with restrictive cardiomyopathy (RCM) and hypertrophic cardiomyopathy (HCM) were previously considered poor candidates for mechanical circulatory support due to technical limitations related to restrictive ventricular physiology and small ventricular size, limiting the ability to provide adequate flows and decompress the heart. Literature examining use of extracorporeal membrane oxygenation (ECMO) in this population consists of a single case series reporting no survivors. We report our experience providing ECMO in children with RCM or HCM at a large pediatric quaternary cardiac center.
