We describe a T-shaped capsule stabilization hook (modified capsule expander [M-CE]) used for repositioning and scleral fixation of the lens capsule of subluxated lenses. The 5-0 polypropylene device is flexible and attached to a curved needle. The contact portion is bent at 1.25 mm, and the end bifurcates in a T configuration to form a 3.75 mm footpad from which the capsular bag can be suspended. Modified capsule expanders were implanted in 4 eyes of 4 patients with subluxated cataractous lenses and provided excellent support and centration of the intraocular lens (IOL)-capsular bag complex. The IOLs remained well centered and stable. The corrected distance visual acuity improved to at least 20/20 in all patients after surgery. Thus, M-CEs were effective in fixating the lens capsule to the sclera in patients with significant zonular weakness.

Purpose: To report a case of disappearance of lens epithelial cells (LECs) detected at the time of treatment for intraocular lens (IOL) dislocation 12 months after cataract surgery.
Observations: A 59-year-old male underwent phacoemulsification with implantation of a posterior chamber acrylic IOL for posterior subcapsular cataract without any complications. Twelve months after cataract surgery, the IOL was dislocated inferiorly from the capsular bag due to rubbing the eye strongly, without capsular deviation.

Purpose: This report describes a surgical approach involving a 5-layer closure performed for a patient with uveitic glaucoma who had an Ahmed Glaucoma Valve placed 12 years prior to presentation, then erosions which were revised 2 and 4 years ago. Additionally, the patient had a Descemet Stripping Endothelial Keratoplasty (DSEK) completed 4 years prior to presentation. The patient presented with a tube that was eroded and a DSEK that had failed.

A 65-year-old man had uneventful cataract surgery in the right eye with a toric diffractive intraocular lens (IOL) placed fully within the capsule bag. On postoperative day 1 and week 1, the IOL was well positioned and his eye was healing normally. The plan was to proceed with cataract surgery in the left eye in the near future.

Background: Dislocated IOL exchange conventionally involves manipulation within the anterior chamber which risks secondary injury to anterior chamber structures. We describe and evaluate a 4-haptic IOL rescue technique that avoids entering the anterior chamber and thus minimizes post operative inflammation, astigmatism and recovery time relative to conventional IOL explantation and replacement techniques.
Methods: Retrospective, non-randomized, interventional study of all patients undergoing 4-haptic IOL rescue performed by two independent vitreoretinal surgeons at a single UK centre over two years.
