Therapeutic endoscopy is mostly primary method in treatment of biliary tree obstruction with its various complications. A case review describing an oncological pacient examined for obstructive icterus, with recognised pneumoscrotum after endoscopic examination (ERCP). In discussion we try to point out the need of knowledge of clinical examination, clinical signs of duodenal lesions and optimal treatment measures.

Pneumoscrotum is the term used to indicate the presence of air in the scrotum and comprises scrotal emphysema and pneumatocele. It is an uncommon medical condition and encompasses multiple etiologies, some of which may be life-threatening. We present the case of a 45-year-old male who developed a pneumoscrotum seven days after undergoing a thoracoscopy with decortication, pleural biopsy, and chest tube insertion, for a loculated pleural effusion not amenable to drainage by a pigtail catheter.

Pneumoperitoneum in neonates is not an uncommon condition. Free air in peritoneum may be secondary to host of pathological lesions. Usually the patient presents with signs of intraperitoneal sepsis, however presence of air in the scrotum as a concomitant sign is a rare event.
