Article Abstract

Perilunate dislocations and fracture dislocations represent uncommon and unusual injuries that are often missed at initial presentation and diagnosed late in up to 25% of cases. Prompt open reduction, carpal stabilisation and ligamentous repair is required to reduce the risk of complications. We report a case of an established scaphoid non-union in an operatively managed perilunate fracture dislocation that spontaneously united almost 2 years after the initial injury, just before a planned revision scaphoid fixation with bone grafting. This case highlights the importance of initial clinical assessment together with appropriate radiographs and follow-up of these injuries post-operatively, especially when complications such as non-union arise.

Background: Post-radiation fractures (PRF) are a recognised complication of radiation treatment for soft tissue sarcomas. They have a low incidence and typically occur up to 5 years following treatment, more commonly affecting the pelvis, ribs and femur. Due to radiation-induced changes in bone, PRFs typically require more complicated intervention compared to post-trauma fractures, however, limited literature exists, particularly in regards to mid-shaft femoral PRFs.

Conservative treatment of clavicle fractures has the disadvantage of substantial pain and discomfort, whereas internal fixation of these fractures has a high rate of non-union. External fixation is an alternative method permitting early mobilization without disrupting the surrounding soft tissue envelope. These are extremely beneficial in young, athletic and highly functional patients as well as in patients with comminuted fractures.

Patellar fractures can lead to extensor mechanism dysfunction if not repaired properly, impacting knee function and mobility, and this complication can be challenging to manage, especially in cases where previous surgical interventions have failed. The aim of this study was to evaluate the functional outcome of the salvaged extensor mechanism in patella fracture non-union using Krakow sutures in a 61-year-old female patient who presented with knee pain, reduced flexion, and a 40-degree extension lag after two previous patellar fracture surgeries using tension band wiring resulted in non-union. Despite rehabilitation attempts, her knee function remained compromised.
