The evolution to a bipedal mode of locomotion was accompanied by a verticalization of the spine and a modification in the shape of the pelvis: horizontal curvature and sagittal rotation. Phylogenesis meets ontogenesis: flat bones in fetuses similar to the monkey, australopithecus features at birth and "human-like" features by 7 or 8years of age. These anatomical modifications explain the characteristics of human bipedalism: stable, economical, with hip and knee extension in the standing position with little lateral motion. Some pathologies induce a regression to a more archaic mode of bipedal locomotion.

The term "fragility fractures of the pelvis" refers to the disruptions of the pelvic ring that are caused by low energy injuries (such as low-level falls or falls from the standing position) in the elderly population (age over 65 years) in the absence of metastatic bone disease. These fractures are increasing in numbers, due to the aging population, particularly in the developed countries, causing significant morbidity and mortality [1]. Although some fracture patterns are stable enough requiring only conservative treatment, other fracture types can cause significant pelvic instability, demanding a more insistent management protocol.

Sclerosing epithelioid fibrosarcoma (SEF) is a rare and aggressive neoplasm composed of epithelioid cells arranged in strands and nests embedded in a highly sclerotic collagenous stroma. We report a case of a 36-year-old man who started with lumbar pain, with extension to both legs, night sweats, and weight loss. He underwent magnetic resonance imaging (MRI) of the lumbar spine; computed tomography (CT) scan of the chest, abdomen, and pelvis; and [18F]-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) scan.

Physical examination revealed palpable lesions in muscles of several extremities. Ultrasound examination confirmed the presence of round lesions with a hypo- or anechoic centre within the muscles.
