We present a 28-year-old patient with liquefying idiopathic lobular panniculitis, a variant of the Weber-Christian's disease and intermittent episodes of arthritis in the left foot refractory to sequential treatment with anti-inflammatory drugs, high doses of prednisone, chloroquine, dapsone, colchicine, cyclosporine and methotrexate.

A 34 year's old female patient died of nodular liquefying panniculitis after a six months evolution. The only systemic lesions found at post mortem histological examination were an involvement of both pleural cavities with an oily fluid. This seemed to have been the cause of death.

A patient with nodular liquefying panniculitis, polyarthritis and an occult pancreatic neoplasm is described. The skin lesion was initially mistaken for erythema nodosum. Subcutaneous and synovial fluids demonstrated similar negatively birefringent, rectangular particles, lying in between fat globules.
