Hearing results and extrusion rate for 89 consecutive patients receiving hydroxylapatite prostheses for hearing reconstruction were evaluated and compared to a control group of 75 patients who had received homograft bone or Plasti-Pore prostheses. At the 3-month follow-up, there were no cases of extrusion of the hydroxylapatite prostheses. Hearing success was defined as a postoperative air-bone gap of less than or equal to 15 dB for incus prostheses and partial ossicular replacement prostheses or less than or equal to 25 dB for incus-stapes prostheses and total ossicular replacement prostheses. There was a statistically significant difference in the distribution-of-success rate by prosthesis type for the hydroxylapatite. Incus, incus-stapes, and total ossicular replacement prosthesis results were similar, but partial ossicular replacement prosthesis results were poorer. The overall success rate in the hydroxylapatite group was 51.2%; in the control group, it was 60%. Hearing results tended to be better with the hydroxylapatite incus prosthesis than with the homograft bone incus, but poorer with the hydroxylapatite partial ossicular replacement prosthesis than with the Plasti-Pore. Several technical modifications of the prosthesis that may improve results with the hydroxylapatite partial ossicular replacement prosthesis are suggested. Hydroxylapatite middle ear prostheses appear to be a reasonable alternative to other more commonly used prostheses, which carry disadvantages related to the use of human tissue.

In this study, a method for determining the optimal location and orientation of an implantable piezoelectric accelerometer on the short process of the incus is presented. The accelerometer is intended to be used as a replacement for an external microphone to enable totally implantable auditory prostheses. The optimal orientation of the sensor and the best attachment point are determined based on two criteria-maximum pressure sensitivity sum and minimum loudness level sum.

Aim: To evaluate the audiometric results of tympanoplasty with stapedectomy and insertion of a synthetic total ossicular replacement prosthesis (TORP).
Material And Methods: Retrospective observational study conducted on a cohort of 15 patients (16 ears) aged 10 to 58 years (mean: 36 years) with chronic otitis media (COM) and tympanosclerosis (n=9), stapediovestibular ankylosis (n=3), minor aplasia (n=3) or post-traumatic ossicular dislocation (n=1). Treatment consisted in tympanoplasty with stapedectomy and TORP placement by the same operator, between December 1, 2012 and January 30, 2023, in a French university hospital department, with follow-up ranging from 2 to 92 months (mean: 24 months).

Retrospective analysis of clinical data of 123 patients with atticotomy, exploring the clinical characteristics of patients undergoing atticotomy and the efficacy of hearing reconstruction methods. 123 patients with atticotomy were divided into three groups according to the ossicular chain treatment method: preservation of the ossicular chain group(37 cases), cartilage elevation of stapes group(49 cases), and PORP group(37 cases). The clinical characteristics of patients with atticotomy, preoperative and postoperative hearing levels of the three groups of patients, and postoperative complications were analyzed.
