Background: Carbohydrate-deficient transferrin (CDT) is used as a marker for chronic alcohol abuse. The presence of genetic transferrin variants might affect an individual's iron status and can interfere with CDT analysis. We report on the identification of a patient carrying a novel transferrin variant. We describe the performance of the various CDT methods in its detection and the associated iron status.

Methods: DNA of the coding region of transferrin was sequenced and CDT levels were analysed using four different methods: high pressure liquid chromatography (HPLC), capillary zone electrophoresis (CZE), immunochemistry and iso-electric focussing (IEF).

Results: A novel transferrin variant, T139M, was found as a heterozygous genotype in the index patient and all of his four living direct family members (c.416C>p.Thr139Met). CDT analysis of the variant by HPLC and CZE was compromised as a result of the coelution of the different isoforms. CDT levels could be quantified by immunochemistry. Similar results were obtained using IEF analysis. The presence of the C2 transferrin variant did not affect iron status in any of the investigated patients.

Conclusions: Transferrin T139M, present as a heterozygous genotype, interferes with CDT analysis by HPLC and CZE but not by immunochemistry. Physiologically, it appears to be functionally normal.

Effect of iron status on myocardial infarction: A two-sample Mendelian randomization study.

December 2024

Department of Geriatrics, Affiliated Hospital of Nanjing University of Chinese Medicine, Jiangsu Province Hospital of Chinese Medicine, City Nanjing, China.

Background: In observational studies, connections have been identified between iron status and myocardial infarction (MI). The significance of changes in iron status as either a risk factor or a result of MI remains unclear.

Methods: We obtained our instrumental variables from a meta-analysis of three GWASs in Iceland, the UK, and Denmark, which discovered 62 independent sequence variants across 56 loci linked to blood iron levels, ferritin, total iron binding capacity (TIBC), transferrin saturation (TSAT), and the Genetics of Iron Status (GIS) database for transferrin.

Transferrin Receptor (TfR)-mediated transcytosis across the blood-brain barrier (BBB) enables the uptake of bispecific therapeutic antibodies into the brain. At therapeutically relevant concentrations, bivalent binding to TfR appears to reduce the transcytosis efficiency by receptor crosslinking. In this study, we aimed to improve BBB transcytosis of symmetric antibodies through minimizing their ability to cause TfR crosslinking.

Controversial role of lactoferrin in cancer: A narrative review.

Lactoferrin (Lf) is a positively charged iron-binding glycoprotein that has piqued the scientific community's interest due to its pleiotropic behavior, exhibiting a wide range of biological activities, including antimicrobial, antioxidant, anti-inflammatory, immunomodulatory, and anticancer effects. This narrative review explores the current understanding of Lf's role in cancer, focusing on the endogenously expressed human full-length and ΔLf isoforms, and the effects of treatment with exogenous human and bovine Lf. We evaluated and compared the mechanisms by which Lf influences tumorigenesis and cancer progression, focusing on its impact on key processes such as cell growth, apoptosis, angiogenesis, cell migration, and invasiveness.

Objectives: To test whether haemochromatosis C282Y homozygotes have increased risk of diabetes, liver disease, and heart disease even when they have normal plasma iron, transferrin saturation, or ferritin concentrations and to test whether C282Y homozygotes with diabetes, liver disease, or heart disease have increased mortality compared with non-carriers with these diseases.

Design: Prospective cohort study.

Setting: Three Danish general population cohorts: the Copenhagen City Heart Study, the Copenhagen General Population Study, and the Danish General Suburban Population Study.

Early Onset of Wilson's Disease and Possible Role of Disease-Modifying Genes: A Case Report and Literature Review.

Wilson's disease (WD) is a rare autosomal recessive disorder caused by mutations in the ATP7B gene, resulting in copper accumulation. Symptoms rarely appear before the age of 5, almost never before 3. The phenotypic variability of WD suggests the presence of modifying factors, making early diagnosis challenging.
