Syringoceles are cystic dilations of the bulbourethral gland, also known as Cowper's gland. Syringoceles are a rare pathology with no standard treatment. Herein, we report the diagnosis and endoscopic management of an imperforate syringocele causing bothersome urinary symptoms in an adult male patient.

Cowper's syringocele is a cystic dilation of the bulbourethral duct or gland. This rare pathology has historically been diagnosed in the pediatric population, with recent literature reporting an increased incidence in adults. Attempts have been made to classify Cowper's syringoceles by their appearance on imaging and endoscopy, however a simpler classification of unobstructed versus obstructed may be of more utility in directing management.

Rationale: Cystic dilatation of the bulbourethral gland duct (Cowper's syringocele, CS) is a rare urethral pathology. No more than 150 cases of CS have been reported in the literature, of which the vast majority are children with a unilateral location. Bilateral CS has been reported in eight cases; however, detailed anatomy and clinical manifestations have not been reported.

Article Synopsis
  • The Cowper glands and ducts are usually undetectable on imaging, but abnormalities are becoming more common due to improved imaging techniques.
  • This article reviews the anatomy of these glands and the imaging findings related to various conditions, including Cowper duct syringocele, Cowperitis, and Cowper stones.
  • It highlights the diagnostic roles of retrograde urethrography and MRI, and mentions the rarity of Cowper gland carcinoma and hyperplasia, which can sometimes mimic cancer in imaging results.
