Respiratory distress is usually a life-threatening emergency in any species and this is particularly important in avian species because of their unique anatomy and physiology. In the emergency room, observation of breathing patterns, respiratory sounds, and a brief physical examination are the most important tools for the diagnosis and treatment of respiratory distress in avian patients. These tools will help the clinician localize the lesion. This discussion focuses on the 5 anatomic divisions of the respiratory system and provides clinically important anatomic and physiologic principles and diagnosis and treatment protocols for the common diseases occurring in each part.

Background: Fluid management is a crucial critical care component, influencing outcomes such as organ function, renal integrity, and survival in critically ill patients. Recent evidence suggests that balanced crystalloids may offer advantages over isotonic saline, particularly in reducing the risk of acute kidney injury (AKI) and other complications. This study aimed to evaluate the impact of balanced crystalloids versus isotonic saline on clinical outcomes in the intensive care unit (ICU), focusing on AKI, renal replacement therapy (RRT), and mortality.

This study investigated the relationship between maternal serum amyloid A (SAA) levels, a biomarker of systemic inflammation, and specific neonatal outcomes in preterm birth (PTB). The study included 66 consecutive pregnant women hospitalized for spontaneous preterm delivery (ranging from 28 to 36 gestational weeks), at the Timisoara Municipal Hospital. The study measured mSAA levels to assess their potential as predictors of fetal outcomes (respiratory distress syndrome [RDS]), as well as their association with APGAR score, neonatal leukocyte count, and C-reactive protein (CRP) levels as indicators of neonatal status and response.

Tetralogy of Fallot with absent pulmonary valve and a nonconfluent left pulmonary artery is an uncommon combination usually associated with respiratory distress due to tracheobronchial compression by the aneurysmally dilated right pulmonary artery. Although application of the conventional technique is not feasible for this subset of patients, our novel "spiral plication" technique, wherein both the anterior and posterior walls of the dilated right pulmonary artery are resected in a spindle shape and plicated as if it is squeezed spirally, provides effective release of the obstructed airway concomitant with reconstruction of the left pulmonary artery confluence with growth potential.

Background: Primary graft dysfunction (PGD) is the leading cause of short- and long-term mortality associated with lung transplantation. The impact of pretransplantation blood transfusions for recipients is not fully elucidated.
Methods: This is a retrospective review of 206 consecutive lung transplantations performed at a single academic center (Northwestern University Feinberg School of Medicine, Chicago, IL) from January 2018 to July 2022.

Background: Candidacy for venovenous extracorporeal membrane oxygenation is dictated by ECMO to Rescue Lung Injury in Severe ARDS (EOLIA) criteria. We evaluated the effect of modifying candidacy on the basis of escalating demand and limited resources.
Methods: We retrospectively reviewed adult patients diagnosed with COVID-19-related severe acute respiratory distress syndrome who failed to respond to conventional ventilation and required extracorporeal support at our institution.
