Background: Spinal melanocytomas are variants that can be extradural or intradural and are most often located in the intradural extramedullary compartment of the main thecal sac as in schwannomas. However, origin of this variant from the pure peripheral compartment of rootlets is exceedingly rare.
Case Description: The authors present a case of spinal melanocytoma with confusing pathologic and radiologic features. This patient presented clinically with severe radiating pain on the right lower extremity. Before surgery, clinical and radiologic findings were consistent with a schwannoma. However, on operation, a coal-black pigmented lesion mimicking old blood clot was found inside the right S-1 root sheath, attached to the dura. The mass was completely removed and consequently the patient's symptoms improved.
Conclusions: The spinal melanocytoma should be included as a differential diagnosis before giving a presumptive diagnosis of schwannoma or hemorrhage for the patient with a space-occupying lesion at a peripheral rootlet. Intraoperative gross morphology and histopathologic findings facilitate differential diagnosis.

Dihydrotestosterone (DHT), an androgen derivate, is known to be a key factor involved in androgenetic alopecia. DHT suppresses the growth of outer root sheath cells and induces apoptosis of hair keratinocytes, thereby causing hair follicle miniaturization and hair regrowth inhibition. Forsythoside A, a natural substance derived from Forsythia suspensa, has been shown to reduce DHT-induced apoptosis in human hair cells and suppress hair regrowth inhibition induced by DHT in mice.

Cytochrome P450s of the CYP79 family catalyze two N-hydroxylation reactions, converting a selected number of amino acids into the corresponding oximes. The sorghum genome (Sorghum bicolor) harbours nine CYP79A encoding genes, and here sequence comparisons of the CYP79As along with their substrate recognition sites (SRSs) are provided. The substrate specificity of previously uncharacterized CYP79As was investigated by transient expression in Nicotiana benthamiana and subsequent transformation of the oximes formed into the corresponding stable oxime glucosides catalyzed by endogenous UDPG-glucosyltransferases (UGTs).

Malignant proliferating trichilemmal tumors (MPTTs), arising from the external root sheath of hair follicles, are exceptionally rare, with limited documentation of their genetic alterations. We present a case of a 64-year-old African American woman who initially presented with a gradually enlarging nodule on her posterior scalp. An initial biopsy at an outside hospital suggested metastatic adenocarcinoma or squamous cell carcinoma (SCC) of an uncertain origin.
