Eclampsia is a multisystem disorder of pregnancy and the puerperium. Posterior reversible encephalopathy syndrome (PRES), a neurotoxic condition characterized by various neurological symptoms, can arise from multiple causes including eclampsia. Unlabelled: Haemolysis, elevated liver enzymes and low platelets (HELLP) syndrome is a poorly understood, life-threatening multisystemic condition related to pregnancy with a rapid onset, typically observed in patients with severe pre-eclampsia. Various mechanisms may lead to diffuse endothelial damage associated with HELLP and possible brain involvement. A comprehensive review of PubMed, Embase and Cochrane databases was conducted to examine the clinical, laboratory and radiological features associated with postpartum HELLP syndrome, particularly its potential association with posterior reversible encephalopathy syndrome (PRES).

Background: New onset seizures in pregnancy and postpartum are considered eclampsia as the default diagnosis. Other causes, such as intracranial haemorrhage, subarachnoid haemorrhage, cerebral venous sinus thrombosis or acute ischaemic stroke, have a similar mode of presentation, but need prompt diagnosis and institution of specific measures to decrease the risk of mortality and morbidity. Neuroimaging plays an important role in providing a specific diagnosis and hence aids in proper management.
