Unstable Ankle Fractures in Neuropathic Diabetics.

J Am Acad Orthop Surg

December 2022

From the Loyola University Health System, Loyola University Medical Center, Maywood, IL.

Connolly recognized as early as 1998 that a displaced ankle fracture in a neuropathic diabetic puts that patient at risk for an amputation. It is well appreciated that the risk of a poor clinical outcome secondary to failure of the surgical construct, deep wound infection and osteomyelitis or a combination of both, is greatly increased in neuropathic diabetic patients, that is, those that are insensate to the Semmes-Weinstein 5.07 (10 g) monofilament, as compared with sensate diabetics or similar non-diabetic patients.

Insensate flaps are used in several reconstructions. A search of the literature showed that most studies are limited to particular flaps in specific body areas. There is a lack of uniform scientific data on the pattern of sensory recovery in various body parts for different kinds of flaps.

There are only a few studies that have shown an association of peripheral neuropathy with cognitive impairment in elderly individuals. Therefore, we investigated the relationship between cognitive performance and peripheral neuropathy. From the database of the National Health and Nutrition Examination Survey (NHANES, 1999-2002), each participant completed a household interview, physical performance test, questionnaire regarding personal health, and Digit Symbol Substitution Test (DSST) to evaluate cognitive performance.

Immediate targeted nipple-areolar complex re-innervation: Improving outcomes in immediate autologous breast reconstruction.

J Plast Reconstr Aesthet Surg

July 2021

Division of Plastic and Reconstructive Surgery, Stanford University, 770 Welch Road, Suite, Palo Alto, CA 94304, United States. Electronic address:

Breast reconstruction often renders the chest skin and nipple areolar complex (NAC) insensate. We propose a new technique of preserving the intercostal nerves during mastectomy and using them to reinnervate the NAC following mastectomy and immediate autologous tissue reconstruction. The technique involves preservation of the lateral intercostal nerves during mastectomy, dissection of the lateral intercostal nerves to length, coaptation of the intercostal nerves to a nerve graft which is then tunneled through the free flap and the distal nerve graft is then coapted to the nerve stumps at the base of the NAC.
