Since the 'Diabetes Complications and Control Trial' results were published in 1993, intensive insulin therapy has been the standard treatment for paediatric patients with insulin-dependent diabetes mellitus. This therapy significantly lowers the risk of long term complications of diabetes. The intensive therapy occurs by either continuous insulin infusion via a pump or a 4-times daily injection regime. In three young patients--two girls of 16 and a boy aged 12 years--the intensive treatment option appeared to be inadequate due to a variety of factors including lack of parental support and an eating disorder. All patients were put on a 2 or 3-times daily regime, which resulted in great improvements to their HbA1c levels and sense of well being. To the boy and one of the girls, extra support was given by a home care nurse. We recommend an individualized approach to the paediatric diabetic patient when it comes to the degree of intensity of the treatment.

Background: Nasal high flow (NHF) has been proposed to sustain high intensity exercise in people with COPD, but we have a poor understanding of its physiological effects in this clinical setting.
Research Question: What is the effect of NHF during exercise on dynamic respiratory muscle function and activation, cardiorespiratory parameters, endurance capacity, dyspnoea and leg fatigue as compared to control intervention.
Study Design And Methods: Randomized single-blind crossover trial including COPD patients.

Nasal high flow (NHF) therapy is an established form of non invasive respiratory support used in acute and chronic care. Recently, a new high flow nasal cannula with asymmetric prongs was approved for clinical use. The clinical benefits of the new cannula have not yet been defined and no evidence are available on the use of asymmetric NHF support in patient with Chronic Obstructive Pulmonary Disease (COPD).

Background/purpose: Immunotherapies, such as CAR-T, have revolutionized cancer treatment for some cancers. However, these treatments often require active participation of a family member or friend to act as a caregiver at home for several weeks after infusion. Given the novelty of CAR-T, there is a need to better understand the experience of patients receiving these treatments and their caregivers.

Objectives: The objectives were to determine intensive care unit (ICU) incidence of broncho-aspiration (BA) and the effect of monitoring BA prevention protocols.
Methods: The Health Network Warehouse was interrogated for the diagnosis of BA in patients older than 18 years in the surgical ICU (SICU) from January 2010 to December 2020. A BA prevention bundle protocol was prospectively monitored during all consecutive SICU admissions from August 2021 to November 2021 until discharge/death (n = 159).

Background: The objective was to investigate if cryoneurolysis were superior to sham in reducing pain intensity in patients with chronic knee osteoarthritis. We hypothesized that cryoneurolysis was an effective and safe therapy to reduce chronic pain in patients with knee osteoarthritis.
Methods: The study was a randomized, double-blind, sham-controlled.
