This case highlights the effectiveness of 177Lu-PSMA therapy in a 70-year-old man with castration-resistant prostate cancer who has extensive skeletal metastases in posttherapeutic whole-body imaging. A 70-year-old man with extensive skeletal metastases underwent 4 cycles of 177 Lu-PSMA-617 therapy. A whole-body 177Lu-PSMA scan after the first cycle showed diffuse skeletal lesions, with minimal PSMA uptake in the kidneys and salivary glands.

Background/objectives: The superscan pattern is a characteristic finding on bone scintigraphy, associated with a variety of metabolic bone diseases, malignancies, and other conditions. This pattern is characterized by a diffuse and intense uptake of radiotracer throughout the entire skeleton. Despite being a relatively rare finding, the superscan pattern can have significant clinical implications.

We herein present a patient initially suspected of multiple lytic skeletal metastasis of unknown primary on anatomical imaging. Metabolic imaging by [18F]-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) detected focal [18F]FDG uptake in the right thyroid nodule, mild [18F]FDG uptake in soft tissue lesion in the left inferior parathyroid region, and multiple nonavid osteolytic skeletal lesions. Fine-needle aspiration cytology of the right thyroid nodule showed papillary thyroid carcinoma (PTC).

We herein describe a rare case of adult acute lymphoblastic leukemia with an florine-fluorodeoxyglucose (F-FDG) positron emission tomography/computed tomography (PET/CT) skeletal superscan-like appearance. The degree of bone marrow uptake was so intense that it far exceeded the level of physiological cerebral uptake and radiourinary activity. The distribution was remarkably similar to a superscan seen on skeletal scintigraphy.
