Non-conventional snake venom toxins, such as WTX from the cobra Naja kaouthia, are three-finger proteins containing a fifth disulfide bond in the N-terminal polypeptide loop I and inhibiting α7 and muscle-type nicotinic acetylcholine receptors (nAChRs). Because the central polypeptide loop II of non-conventional toxins plays an important role in their biological activity, we synthesized several WTX loop II fragments with two cysteine residues added at the N- and C-termini and oxidized to form a disulfide bond. The inhibition by peptides of several nAChRs subtypes was investigated using different methods and the effects of peptides on the rat arterial pressure and heart rate were analyzed.

Purpose: Middle-segment preserving pancreatectomy (MSPP) serves as an alternative to total pancreatectomy (TP) for preserving the pancreatic body in multifocal pancreatic neoplasms. Despite the potential benefits of TP, the detailed short- and long-term prognoses remain unclear. We evaluated the feasibility of MSPP by comparing the perioperative outcomes and postoperative endocrine and exocrine functions with those of TP.

Introduction: Mitral regurgitation is a potential complication of transcatheter aortic valve replacement. Here, we report a case of severe acute mitral regurgitation caused by papillary muscle rupture occurring 16 days after transcatheter aortic valve replacement.

Presentation Of Case: An 82-year-old woman with severe AS was referred to our hospital.

Article Synopsis
  • * A case study described a 39-year-old man who experienced severe chest pain due to a heart attack linked to a PFE found on imaging, which showed a mass near his aortic valve.
  • * The patient successfully underwent surgery to remove the tumor, and the case underscores the need for advanced imaging techniques to identify these rare tumors, as well as the urgency of surgery when they cause symptoms.

Impella 5.5 as a bridge-to-surgery in acute ischemic mitral regurgitation post-percutaneous coronary intervention: a case report.

J Cardiothorac Surg

November 2024

Division of Advanced Heart Failure and Cardiac Transplant, University of South Florida, Tampa General Hospital, 7110 S Trask Street, Tampa, FL, 33616, USA.

Article Synopsis
  • Acute ischemic mitral regurgitation (AIMR) is a serious condition that can occur during acute coronary syndrome, leading to rapid heart failure and shock; intra-aortic balloon pumps (IABP) are often used to stabilize patients before surgery.
  • A 74-year-old man with cardiac issues underwent an initial stenting procedure but continued to experience severe hypoxia and hypotension, prompting the upgrade to an Impella device for better circulatory support.
  • Although his condition improved enough for surgery to replace the mitral valve and perform bypass grafting, he later developed bacterial pneumonia and acute respiratory distress syndrome, leading to his death shortly after.
