Introduction: Amiodarone-induced thyrotoxicosis (AIT) is a common clinical disorder that may be life threatening and whose clinical manifestations and response to treatment may vary among patients.

Methods: We present three patients treated with amiodarone for atrial fibrillation who developed AIT at least 36 months after beginning the treatment. Thyrotoxicosis worsened the underlying cardiac disorders and was resistant to treatment based on the combination of dexamethasone 8-12 mg/day i.v., thioamides 45 mg/day p.o., beta blockers and potassium perchlorate at doses of 800 to 1000 mg per day p.o. Two of the patients attained sustained euthyroidism after 12 and 32 days of combined treatment, while the third required total thyroidectomy.

Conclusion: The combination of thioamides with potassium perchlorate is an appropriate form of therapy for AIT in patients resistant to thioamides. The use of this combination should be evaluated in patients with mixed AIT or AIT of unclear etiology.

Utility of ultra-portable ultrasound devices for the risk-stratification of thyroid dysfunctions in an emergency setting.

J Endocrinol Invest

October 2024

Department of Endocrine and Metabolic Diseases, Istituto Auxologico Italiano IRCCS, Milan, Italy.

Article Synopsis
  • The study examines the effectiveness of rapid TSH screening in detecting thyroid diseases in patients in the Emergency Department (ED), noting a 7% prevalence of previously undiagnosed thyroid dysfunctions.
  • The diagnostic process was enhanced by using a mobile ultrasound device, rPOCUS, to evaluate patients with suppressed rTSH in urgent care situations requiring iodinated contrast.
  • Results indicate that rPOCUS is both cost- and time-effective, aiding in the management of hyperthyroidism, reducing unnecessary rTSH tests, and identifying incidental findings that necessitate further evaluation.

There is increasing concern regarding pollutants disrupting the vertebrate thyroid hormone (TH) system, which is crucial for development. Thus, identification of TH system-disrupting chemicals (THSDCs) is an important requirement in the Organisation for Economic Co-operation and Development (OECD) testing framework. The current OECD approach uses different model organisms for different endocrine modalities, leading to a high number of animal tests.

Ammonium perchlorate (AP) has been widely used as an oxidizer in propellants and military mixed explosives in recent years. However, its high characteristic signal, environmental pollution, and poor detonation performance have prompted the industry to seek alternatives to AP. Ammonium nitrate (AN) is a suitable substitute due to its low characteristic signal, lack of pollution, and excellent detonation performance.

The hydrogenation of metal nanoparticles provides a pathway toward tuning their combustion characteristics. Metal hydrides have been employed as solid-fuel additives for rocket propellants, pyrotechnics, and explosives. Gas generation during combustion is beneficial to prevent aggregation and sintering of particles, enabling a more complete fuel utilization.
