Three-year follow-up of fenestrated thoracoabdominal stent graft bridging an endovascular thoracic stent graft and a surgical abdominal aortic graft.

This case report describes repair of a type I endoleak at the distal landing zone of a thoracic aortic stent graft by endovascular placement of a thoracoabdominal fenestrated stent graft (Cook, Brisbane, Australia). The fenestrated stent graft was interposed between a previous abdominal aortic aneurysm (AAA) Gelsoft tube graft (Sulzer Vascutek Ltd, Inchinnan, United Kingdom) and two overlapping Zenith thoracic endografts (Cook Inc, Bloomington, Indiana). Placement was made more complex because the distal thoracic endograft had rotated into a horizontal position. At 3-year clinical and computed tomography (CT) follow-up, continued clinical and radiologic success was shown with no further intervention required.

Methods: The physician constructed Endo-Bentall device is composed of a self-expanding transcatheter aortic valve (TAVR), aortic endovascular stent graft (TEVAR), and two wire-reinforced fenestrations for coronary artery stenting. The TAVR valve is sutured into an appropriately sized TEVAR graft.

Carotid artery stenting (CAS) has become a favoured alternative to surgical carotid endarterectomy (CEA) in select cases of critical internal carotid artery (ICA) occlusion. However, complications such as stent migration or entrapment can occur, necessitating prompt diagnosis and intervention. We present a case of a 75-year-old diabetic male who underwent CAS for recurrent presyncope at a private hospital, and during the procedure of CAS the stent was migrated and its proximal part was impacted in the critically narrowed part of the ICA.

Fibrosing mediastinitis is a rare chronic inflammatory condition characterized by excessive fibrotic process in the mediastinum. Although various infectious processes are more common causes, it can also be related to several malignancies. We report a case of a 28-year-old woman with fibrosing mediastinitis related to an aggressive primary gray-zone lymphoma causing complete occlusion of the superior vena cava (SVC) and the innominate veins.
