Background And Importance: Endovascular management of dural arteriovenous fistulas (DAVFs) has become an accepted primary and often definitive therapy. We present the first documented case of Onyx pulmonary embolism after embolization of a low-flow DAVF.

Clinical Presentation: A 63-year-old man presented with subarachnoid hemorrhage secondary to a DAVF. Computed tomographic angiography, magnetic resonance imaging, and initial conventional angiogram were negative. A repeat angiogram demonstrated a DAVF, which was cured with Onyx embolization. A routine chest computed tomography after intervention revealed an asymptomatic Onyx embolization cast in a subsegmental pulmonary artery.

Conclusion: Endovascular treatment options include transarterial embolization with microcoils, polyvinyl alcohol particles, n-butyl-2cyanoacrylate, and Onyx (ev3 Neurovascular, Irvine, California). Complications associated with the use of Onyx are low but include embolizate pulmonary embolism. Patients often remain asymptomatic, but for symptomatic patients, conservative treatment options usually result in resolution of symptoms.

Serial Dilation and Intentional Fracturing of Stents in Congenital Heart Disease-Benchside Tests and Initial Clinical Results.

Catheter Cardiovasc Interv

December 2024

Department of Pediatric Cardiology and Congenital Heart Disease, German Heart Center Munich, Technische Universität München, München, Germany.

Background: There is some reluctance to implant stents in small children due to concerns regarding outgrowing the maximal stent diameter during follow-up.

Aim: Evaluation of a treatment strategy on the bench side, including intentional stent fracturing, and description of our initial clinical experience.

Methods: A series of benchside tests was performed with small stents, in which the stents were dilated above the rated diameters until they ultimately fractured.

Article Synopsis
  • - The MATCH-R trial aimed to analyze the resistance mechanisms to cancer treatments by studying fresh tumor biopsies from metastatic patients, covering data collected from 2015 to 2022.
  • - Out of 1,120 biopsies from patients primarily with lung, digestive, and prostate cancers, 30.9% revealed targetable genomic alterations, with EGFR being the most common altered gene.
  • - Among patients with resistance mechanisms, 45% had treatments tailored based on identified mechanisms, resulting in an average of 11 months of additional clinical benefit.

This first-in-human study evaluated safety and efficacy of CD40 agonist MEDI5083 with durvalumab in patients with advanced solid tumors. Patients received MEDI5083 (3-7.5 mg subcutaneously every 2 weeks × 4 doses) and durvalumab (1500 mg every 4 weeks) either sequentially (N = 29) or concurrently (N = 9).

Article Synopsis
  • A study examined the safety and effectiveness of one-month dual antiplatelet therapy (DAPT) in high bleeding risk patients undergoing PCI with Resolute Onyx stents, focusing on differences between Asian and non-Asian patients.
  • The results showed no significant differences in serious ischemic outcomes (like cardiac death or heart attacks) between matched groups from both regions, each with 12% incidence.
  • However, Asian patients experienced significantly fewer major bleeding events (4%) compared to non-Asian patients (9%), despite similar initial bleeding risk profiles after matching.

  • * A total of 205 patients participated, with a 1-year target-vessel failure (TVF) rate of 7.4%, significantly below the performance goal set at 24.5%, and a 3-year TVF rate of 12.1%.
  • * The findings indicate that using a provisional stenting approach for bifurcation lesions with the Resolute Onyx ZES led to positive clinical outcomes, suggesting it
