Telepsychiatry, or videoconferencing in psychiatry has existed for around forty years in a number of countries. Factors contributing to its development include difficulties in physically accessing specialists or in mobilising certain categories of patients (children, elderly or disabled people). Still not widely developed in France, certain technological and demographic evolutions nevertheless make it increasingly necessary.

During the early COVID-19 pandemic, most psychiatric facilities did not admit SARS-CoV-2-positive youth, resulting in prolonged emergency department (ED) boarding and delayed psychiatric care. In response, our hospital enacted a small, single-site, innovative pilot enabling psychiatric admission of SARS-CoV-2-positive patients with lower behavioral acuity to an inpatient medical unit for comprehensive telepsychiatry programming. This study explores the impact of telehealth services, encompassing (1) telestroke, (2) telepsychiatry and addiction treatment, (3) teleconsultation and office visits,(4) tele-intensive care units, (5) telemonitoring postdischarge, (6) ongoing chronic care management, and (7) other remote monitoring, on social need performance outcomes.

While pediatric mental health emergencies are increasing in frequency and severity, psychiatric resources remain concentrated in tertiary care facilities. Telepsychiatry has successfully mitigated these challenges in rural emergency departments (EDs), suggesting potential benefits for urban EDs that lack psychiatric resources. We implemented telepsychiatry in an urban ED to reduce ED length of stay and the need for transferring pediatric patients with mental and behavioral health complaints.
