Background And Purpose: Monocular diplopia is an infrequent but disabling complication following laser peripheral iridotomy, or following cataract surgery. Our purpose is to clarify the intraocular mechanism of monocular diplopia (and binocular triplopia) of physical origin and its relation to the physiology and arrangement of the visual directions of the retinal areas in each eye.
Case Reports: This is a report of one patient who developed monocular diplopia following an "exposed" laser peripheral iridotomy, and of a second patient who developed monocular diplopia due to a swollen opacified central posterior capsule following cataract surgery.
Conclusion: Monocular diplopia of physical origin results from stimulation, by an object of regard, of two separate retinal areas that have two different visual directions due to optical ocular pathology.

Patients with A20 haploinsufficiency (HA20) presenting with central nervous system (CNS) symptoms are rare, and available reports are limited. Here, we describe a patient with HA20, previously followed up as Behçet disease, who presented with CNS symptoms in adulthood. A 38-year-old Japanese male who had been followed up for incomplete Behçet disease at another hospital since 28 years of age presented to our hospital with acute-onset diplopia and persistent hiccups that were severe enough to cause vomiting.

Background And Objective: Radiosurgery can serve as a primary, adjuvant, or salvage treatment modality for cavernous sinus tumors (CST), providing high tumor control. However, particularly with cavernous sinus expansion, there may be insufficient distance from the optic apparatus to perform radiosurgery safely. The internal carotid artery adjacent to the distal dural ring (ICAddr), when enhancing similarly to the CST, can be difficult to delineate, and can lead to over-contouring of target volume near the optic nerve and therefore increasing the risk of radiation-induced optic toxicity.

Purpose: To analyze Tocilizumab (TCZ, an interleukin 6 inhibitor) as a second-line treatment for thyroid-associated ophthalmopathy (TAO).
Methods: In this retrospective observational study, the charts of patients with moderately severe to severe TAO who received intravenous Tocilizumab as a second-line treatment 2020-2023 were reviewed.

Purpose: To describe and evaluate the efficacy and safety of lacrimal sac to conjunctiva reconstructive ostomy for proximal lacrimal system reconstruction.
Methods: This retrospective study included 41 patients (32 cancer and 9 trauma; 36 primary and 5 secondary) who underwent lacrimal sac to conjunctiva reconstructive ostomy. The technique involves creating a direct connection between a surgical opening in the lacrimal sac, which maintains its bony attachments to prevent ostomy occlusion, and the medial conjunctiva (medial fornix, bulbar, and palpebral).
