Out-patient sclerotherapy of idiopathic left-sided varicocele in children and adults.

Between 1983 and 1989, 334 patients with idiopathic left-sided varicocele were referred for transfemoral sclerotherapy; 323 were adults and 11 were children. The standard site of injection of the sclerosing solution (Varicocid-Natriummorrhuat 55 mg, Benzylalkohol 20 mg) was the mid-portion of the spermatic vein. The upper third was injected only in cases where the catheter could not be negotiated further (e.g. abnormalities in the course of the spermatic vein or its drainage into the intrarenal veins). The amount of sclerosing material was predetermined fluoroscopically using contrast material. Occasionally, in cases of high catheter positioning, the Varicocid was injected fractionally. The amount injected was always between 1 and 3 ml. Of 80 patients studied 1 year later, varicoceles were still present in 3 (4%). The best results were achieved in patients with small varicoceles (Grade I), normal testicular volume and normal FSH levels; 29% showed an improvement in sperm count and 27% an increase in sperm motility, while sperm morphology improved in only 7%. The conception rate was 11%.

This is a novel case of idiopathic chylopericardium and chylothorax in a young male who had no significant medical history. He first presented with dyspnea due to idiopathic chylopericardium, which was refractory to medical and surgical treatments, including a medium-chain triglyceride diet, octreotide, and video-assisted pericardial window. The chylopericardium persisted and progressed to concomitant left-sided chylothorax.

We present а rare clinical case of successful surgical treatment - combined left-sided hemicolectomy and cecal resection for colon cancer of two locations in a patient with idiopathic thrombocytopenic purpura, which, in our opinion, is due to radiation exposure is presented. It is shown that the biological effects of long-term ionizing radiation on critical organs can have a diverse nature both in terms of the time of occurrence and in their structure, which requires an individual approach, the involvement of related specialists and the necessary medical preoperative preparation for precise planning of surgical intervention taking into account all possible risks.

A 69-year-old woman with left-sided breast cancer developed elevated creatine kinase levels and muscle weakness in her extremities after treatment with pembrolizumab. The patient was diagnosed with immune checkpoint inhibitor (ICI)-related myositis. Although the patient had no symptoms of dysphagia, we evaluated her swallowing function because esophageal dysfunction is a known complication of idiopathic inflammatory myopathy.

Spinal cord sarcoidosis, an uncommon manifestation of neurosarcoidosis, presents diagnostic and therapeutic challenges because the condition is rare and has diverse clinical manifestations that can mimic other conditions such as multiple sclerosis and neuromyelitis optica spectrum disorder. A middle-aged African American female with a history of idiopathic intracranial hypertension and hydrocephalus with ventriculoperitoneal shunt presented with progressive, predominantly left-sided gait instability, weakness, and paresthesia. Cerebrospinal fluid showed lymphocytosis, red blood cells, elevated oligoclonal bands, and elevated kappa free light chains, concerning for multiple sclerosis.
