Objective: To quantify and improve the performance of standard rheumatoid arthritis (RA) algorithms in a biobank setting.

Methods: This retrospective cohort study within the Mayo Clinic (MC) Biobank and MC Tapestry Study identified RA cases by presence of at least two RA codes OR positive anti-cyclic citrullinated peptide antibodies (CCP) plus disease-modifying anti-rheumatic drug (DMARD) prescription as of 7/18/2022. Rheumatology physicians manually verified all RA cases using RA criteria and/or rheumatology physician diagnosis plus DMARD use.

Background: Rheumatoid arthritis (RA) is a chronic autoimmune disease that causes joint damage and progressive destruction of adjacent cartilage and bones. Quick and accurate detection of rheumatoid factors (RF) and anti-cyclic citrullinated peptide antibodies (anti-CCP) in serum is effective in diagnosing RA and preventing its progression. However, current methods for detecting these two biomarkers are costly, time-consuming, labor-intensive, and require specialized equipment.

We report the case of a patient who experienced rheumatic immune-related adverse events (irAEs) with anti-cyclic citrullinated peptide (anti-CCP) positivity and cutaneous adverse drug reactions. A 59-year-old woman with malignant melanoma who was treated with nivolumab as postoperative adjuvant therapy developed polyarthralgia with anti-CCP positivity and dermatologic adverse events caused by celecoxib or lansoprazole. Her rheumatic and cutaneous symptoms greatly improved with oral glucocorticoid treatment.
