Toward Developing Alternative Opioid Antagonists for Treating Community Overdose: A Model-Based Evaluation of Important Pharmacological Attributes.

Clin Pharmacol Ther

January 2025

Division of Applied Regulatory Science, Office of Clinical Pharmacology, Office of Translational Sciences, Center for Drug Evaluation and Research, Food & Drug Administration, Silver Spring, Maryland, USA.

In response to increased illicit use of synthetic opioids, various μ-receptor antagonist formulations, with varied pharmacological characteristics, have been and are being developed. To understand how pharmacologic characteristics such as absorption rate and clearance rate affect reversal in treating community opioid overdose, we used our previously published translational opioid model. We adapted this model with in vitro receptor binding data and clinical pharmacokinetic data of three intranasal nalmefene formulations along with an intranasal naloxone formulation to study the reversal of fentanyl and carfentanil-induced respiratory depression in chronic opioid users.

Environmental sustainability and the limits of healthcare resource allocation.

Bioethics

January 2025

Ethox Centre, Nuffield Department for Population Health, University of Oxford, Oxford, UK.

Recent literature has drawn attention to the complex relationship between health care and the environmental crisis. Healthcare systems are significant contributors to climate change and environmental degradation, and the environmental crisis is making our health worse and thus putting more pressure on healthcare systems; our health and the environment are intricately linked. In light of this relationship, we might think that there are no trade-offs between health and the environment; that healthcare decision-makers have special responsibilities to the environment; and that environmental values should be included in healthcare resource-allocation decisions.

Background: Housecleaning work has been characterized as precarious employment with unstable work hours, arbitrary and low pay and benefits, and exposures to chemical, physical, and psychosocial stressors. Understanding how interpersonal power dynamics between workers and clients, a component of precarious work, contributes to work exposures can inform and improve prevention programs.

Methods: We used reflexive thematic analysis of data from seven focus groups with Latinx immigrant housecleaners in New York City to explore workers' experience of interpersonal power dynamics with their clients-whom they referred to as their "employers"-and its influences on working conditions.
