Sigmoid volvulus during pregnancy is an extremely rare condition that presents as intestinal obstruction and can lead to severe complications for both the mother and fetus if not promptly diagnosed and treated. To our knowledge, this is the first documented case in Yemen. We report a case of a 39-year-old pregnant woman at 32 weeks of gestation who presented with acute abdominal pain, constipation, and signs of peritonitis.

It is commonly associated with chronic constipation, particularly in patients with neurogenic bowel disorders or prolonged opioid use. Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract, frequently characterized by mutations in the KIT or PDGFRA genes. This case report details the complex clinical course of a 71-year-old female with a history of HIV and metastatic GIST presenting with acute abdominal symptoms indicative of perforated viscus. Initial imaging revealed a massive pneumoperitoneum and a large abdominal mass, necessitating immediate surgical intervention.
