Background: The use of interosseous wire to fix bone grafts is well known. Herein, we describe a technique for fixation of an iliac crest bone graft for nasal augmentation, using a stainless steel wire.

Method: A hole in the cancellous part of the graft guides the wire exactly into a groove in the cortical part, preventing slippage and ensuring rigid fixation. The wire is then threaded through a hollow spinal needle passed underneath the skin envelope; this avoids a dorsal incision and thus minimises scarring, reduces the risk of graft exposure and improves the aesthetic result.

Results: This technique has two distinct advantages: prevention of wire slippage and avoidance of a dorsal nasal incision.

Conclusion: The described method uses an interosseous wire for rigid bone graft fixation, without a dorsal incision. This prevents wire slippage; it also achieves a good cosmetic result by improving the nasal contour via a cantilever effect which raises the nasal tip.

Closed Intramedullary Pinning of Displaced Radial Neck Fracture (Metaizeau Technique).

JBJS Essent Surg Tech

November 2024

Pediatric Hand, Nerve and Microsurgery, Barcelona Children's Hospital, HM Hospitales, Barcelona, Spain.

Background: Radial neck fractures account for 1% of all pediatric fractures and 5% to 10% of pediatric elbow fractures. The mechanism of injury is typically a fall with the elbow in hyperextension and the forearm in supination. A valgus force compresses the radial head against the capitellum, causing a radial neck fracture.

Ulnar nerve neuropathy following pediatric supracondylar humerus fracture fixation with cross pinning poses challenges in management. Despite various treatment strategies, including conservative approaches and early intervention, achieving complete neural recovery remains elusive in some cases. This paper presents a novel approach utilizing supercharged end-to-side anterior interosseous nerve transfer for a 13-year-old patient who experienced persistent ulnar neuropathy after K-wire removal.

Second Toe-to-thumb Transfer Using Intramedullary Screw Fixation after Traumatic Thumb Amputation.

Plast Reconstr Surg Glob Open

August 2024

From the Section of Plastic and Reconstructive Surgery, Department of Surgery, University of Chicago, Chicago, Ill.

Loss of the thumb secondary to trauma leads to significant disability due to the digit contributing 40% of overall hand function. Toe-to-thumb transfer has been a well-studied reconstructive method as a means to restore function after thumb amputation. First described in 1969 by Cobbett and colleagues, toe-to-thumb transfers have undergone several modifications in technique to better improve functional and aesthetic outcomes, including toe wrap-around flaps, trimmed great toe transfer, and second toe transfers.

Medial pivot dislocation of talonavicular joint- A rare injury with literature review.

J Clin Orthop Trauma

April 2024

Department of Orthopaedic Surgery, Jawaharlal Nehru Medical College, Aligarh Muslim University, Aligarh, Uttar Pradesh, India.

Medial pivot dislocations are dislocation of talonavicular joint (TNJ) medially with calcaneum subluxate beneath the talus with intact calcaneocuboid joint and interosseous ligament. These dislocations have subtle incidence and are mostly related to high energy injuries. These dislocations are easy to reduce having good functional outcome if diagnosed early.

Study Design: Cadaveric investigation.

Objective: Rigid internal fixation (RIF) using plates and screws is often not feasible in low and middle-income countries due to limited resources. Interosseous wiring to achieve semi-rigid fixation is often used, but lacks biomechanical force data.
