Gunshot-related myocardial injuries typically result from direct penetrating injury by the traversing bullet. We present a case of gunshot-related myocardial injury in the absence of direct penetrating injury to the cardiac structures and describe alternative mechanisms for ballistic-induced injury.

Objective: To conduct a comparative analysis of cardiovascular system state in emergency workers (EW) of theaccident at the Chornobyl NPP and servicemen (SM) of Ukraine Armed Forces (UAF) who took part in the fightagainst russian military aggression, and to assess the role of military service factors on the development of cardiac pathology.

Background: Foreign bodies that migrate into the heart may include medical devices dislodged from their original location or, rarely, external particles (shrapnel and other foreign bodies) that penetrate the vein, remain intraluminal, and migrate via the venous blood flow to the right heart. Most reported entry sites of these external foreign bodies were in the torso, thigh, or neck; none of them penetrated through a distal extremity of the body. We report a case where shrapnel was found in the right ventricle (RV) following penetrating injury to the hand.

We present the case of a patient who was shot through the heart with an air rifle and presented in a stable condition at our emergency department. At the time of presentation, the bullet was still present within the myocardium. Imaging, management, and outcome are discussed in this report.
