The results of histological investigation of musculo-aponeurotic tissues of lumbar-lateral abdominal portion were analyzed. There was established, that relaxation and atrophy of these tissues had occurred due to nervous structures and vessels cutting with consequent development of postoperative lumbar-lateral hernias. The plastic material fixation to the bearing structures of lumbar-lateral portion of abdominal wall, specifically to posterior wall of m. rectus abdominis aponeurotic sheath and the twelve rib cartilage constitutes the peculiarity of surgical treatment of postoperative lumbar-lateral abdominal hernias. According to the data of investigation, in 119 of the first group of patients operated on there were no recurrences, in the patients of comparative group, operated on using the standard method of alloplasty, the recurrences had occurred in 8.9% of observations.

Objective: To summarize the clinical effect of a single-center retrospective analysis of the contralateral approach with a microscope and tubular retractor system for ipsilateral decompression in patients with lumbar lateral recess stenosis and a narrow spinal canal.
Methods: A total of 25 patients who underwent ipsilateral decompression surgery via a contralateral approach with microscope and tubular retractor system, performed by one surgeon at a single center were retrospectively examined. The width of the lamina fenestration was compared with the preoperative distance from the root of the spinous process to the dorsal articular facet, the bilateral articular facet change in the suprapedicle notch section on CT scan, and with the changes in transverse and sagittal diameters of the canal area on MRI.

Background Context: Transcranial Motor Evoked Potentials (TcMEPs) can improve intraoperative detection of femoral plexus and nerve root injury during lumbosacral spine surgery. However, even under ideal conditions, TcMEPs are not completely free of false-positive alerts due to the immobilizing effect of general anesthetics, especially in the proximal musculature. The application of transcutaneous stimulation to activate ventral nerve roots directly at the level of the conus medularis (bypassing the brain and spinal cord) has emerged as a method to potentially monitor the motor component of the femoral plexus and lumbosacral nerves free from the blunting effects of general anesthesia.
